İnfertilite ve yaşam kalitesi: Sistematik derleme

AMAÇ: İnfertil bireylere tanı işlemleri için uygulanan testler, tedavi seçeneklerine ilişkin karar verememe, tedavi sürecine ilişkin yeterli bilgilendirilmeme, tedaviler ve sonuçları ile ilgili yaşanan anksiyete ve ambivalan duygular, tanı ve tedavi sürecinde yaşanan fiziksel sıkıntılar ve ağrı gibi nedenler infertil bireylerin yaşam kalitesini olumsuz olarak etkilemektedir. Bu derlemenin amacı infertil bireylerde yaşam kalitesi ile ilgili yapılmış ve yayınlanmış çalışmaların gözden geçirilmesi ve sistematik olarak incelenmesidir. GEREÇ VE YÖNTEM: Sistematik derleme türündeki bu çalışmada anahtar kelime olarak “infertility and quality of life”kullanılarak Pubmed, Science Direct ve Medline veri tabanları taranmıştır. Ocak 2014-Şubat 2019 tarihleri arasında uluslararası dergilerde yayınlanmış çalışmalar değerlendirilmiş olup tarama sonucunda 495 çalışma elde edilmiştir. Dahil etme/ dışlama kriterlerine uygun 12 çalışma derleme kapsamına alınmıştır. BULGULAR: İncelenen çalışmaların sonucuna göre infertil bireylerin fertil bireylere göre yaşam kalitelerinin daha düşük olduğu belirlendi. İnfertil bireyler cinsiyet yönünden karşılaştırıldığında infertil kadınların infertil erkeklere göre yaşam kalitesinin daha düşük olduğu belirlendi. Yaş, eğitim, kültürel farklılıklar ve önceki tedavilerden olumsuz sonuç almak gibi faktörlerin yaşam kalitesi üzerine etkili olduğu görüldü. Aynı zamanda tamamlayıcı alternatif tıp yöntemleri ve maneviyatı kullanma gibi başetme mekanizmalarının yaşam kalitesini artırdığı belirlendi. Sonuç: Üreme fonksiyonunu yerine getirememek ve ebeveyn olamamak gibi toplumsal rollerini yerine getiremediğini düşünmek gibi faktörler ve tedavi süresince yaşananların (yoğun ilaç kullanımı, oosit toplanması gibi invaziv işlemlere maruz kalma, semen verme, gebelik sonucunun yarattığı beklenti ve kaygıların artması vb.) etkisiyle yaşam kalitesi düşmektedir. Bu nedenle hemşirelerin, infertil bireylere bakım verirken tanı ve tedavi sürecinin her aşamasında bireylerin yaşam kalitesini değerlendirmesi önemli ve gereklidir.

Infertility and quality of life: A systematic review

OBJECTIVES: The tests performed for diagnostic procedures, lack of decision about treatment options, insufficient information about treatment process, anxiety and ambivalent feelings about the treatment and results, physical problems experienced during the diagnosis and treatment process and pain, negatively affect the quality of life of infertile people. The aim of this study is to systematically review the published studies about quality of life in infertile people. METHODS: In this systematic review; Pubmed, Science Direct and Medline databases were examined by using “infertility and quality of life” as the keyword. Studies published in international journals between January 2014 and February 2019 were evaluated and 495 studies were obtained. 12 studies complying with the inclusion/ exclusion criteria were included in the study. RESULTS: According to the results of the studies, it was determined that infertile people had lower quality of life than fertile people. Infertile women were found to have lower quality of life than infertile males. Factors such as age, education, cultural differences and negative results from previous treatments were found to be effective on quality of life. It was also found that coping mechanisms such as complementary alternative medicine methods and the use of spirituality increased the quality of life. CONCLUSION: Quality of life decreases due to factors such as unable to perform reproductive function and social roles, and experienced during the treatment period (intense medication use, exposure to invasive procedures such as oocyte retrieval, semen administration, increased expectation and anxiety caused by pregnancy, etc.). Therefore, it is important for nurses to assess the quality of life of individuals at every stage of the diagnosis and treatment process while providing care to infertile people.
