Şizofreni hastalarında intihar girişiminin bilişsel işlevler, umutsuzluk ve iç görü ile ilişkisi

Amaç: Bu çalışmanın amacı intihar girişimi olan ve olmayan şizofreni hastalarını bilişsel işlevler, umutsuzluk ve iç görü açısından karşılaştırmak ve intihar girişimini yordayan etkenleri saptamaktır. Yöntem: DSM-IV tanı ölçütlerine göre 50 şizofreni hastası çalışmaya alınmış olup intihar girişimi olan 21 hasta ile intihar girişimi olmayan 29 hasta karşılaştırılmıştır. Çalışmaya alınan hastalar Negatif Belirtileri Değerlendirme Ölçeği (SANS), Pozitif Belirtileri Değerlendirme Ölçeği (SAPS), Calgary Şizofrenide Depresyon Ölçeği (CŞDÖ), İçgörünün Üç Bileşenini Değerlendirme Ölçeği, Beck Umutsuzluk Ölçeği, Beck İntihar Düşüncesi Ölçeği, Stroop Testi ve Wisconsin Kart Eşleme Testi (WKET) ile değerlendirilmiştir. Bulgular: İntihar girişimi olan şizofreni hastalarında intihar düşüncesi, umutsuzluk, iç görü, CŞDÖ puanları daha yüksek bulunmuştur. Bilişsel işlevler değerlendirildiğinde Stroop 4 süresinin, WKET toplam doğru, toplam yanlış, perseveratif tepki, perseveratif hata sayılarının anlamlı derecede daha iyi olduğu saptanmıştır. Sonuç: Çalışmamızın bulgularına göre intihar girişimi olan şizofreni hastalarının yürütücü işlevlerin intihar girişimi olmayanlara göre daha iyi olduğu söylenebilir.

Relationship between suicide attempt and cognitive functions, hopelessness, insight in schizophrenia

Objective: The aim of this study was to compare cognitive functions, hopelessness and insight in schizophrenia patients with and without suicide attempt and determine the predictive factors for suicide attempt. Methods: Fifty patients with DSM-IV diagnosis of schizophrenia were enrolled prospectively to the study, were categorized based on patient and interviewers as suicide attempters and non-suicide attempters. These groups were compared about depression, positive/negative symptoms, insight, hopelessness and cognitive functions. Results: In group suicide attempters' suicide ideation, hopelessness, insight and depression were higher than non-suicide attempters. Neuropsychological performance was better in patients with suicide attempt. Stroop test times were better, WCST perseverative responses, errors were better in patients with suicide attempt. Conclusion: According to our results patients with suicide attempt executive functions are better than patients without suicide attempt.
