Dirençli obsesif kompulsif bozukluklu hastalarda tedaviye olanzapin eklenmesi

Amaç: Serotonin geri alım inhibitörleri (SGİ), obsesif kompulsif bozukluğun (OKB) tedavisinde yaygın olarak kullanılmasına rağmen; hastaların yaklaşık % 40-60'ı bu tedaviye yeterli ölçüde cevap vermemektedir. Çalışmamızda dirençli OKB olarak değerlendirdiğimiz hastalarda tedaviye atipik bir antipsikotik olan olanzapin eklenmesinin sonuçlarını tartışmayı amaçladık. Yöntem: Çalışma grubunu Fırat Üniversitesi Tıp Fakültesi Psikiyatri Kliniğine Ekim 1999 - Mart 2000 tarihleri arasında başvuran ve DSM-IV tanı ölçütlerine göre OKB tanısı konarak en az üç aylık izlemde tek başına SGİ ya da ek olarak anksiyolitik, duygudurum düzenleyicisi ve nöroleptik kullanımıyla hedeflenen tedavi yanıtının alınamadığı 12 hasta oluşturdu. Hastalarda OKB 'un şiddeti Yale-Brown Obsesyon Kompulsiyon Ölçeği (Y-BOKÖ) ile izlenerek tedavi yanıtı alınamayan olgularda doz arttırımına gidildi. Sonuçlar: Hastaların 7'sinde Y-BOKÖ skorlarında belirgin (en az % 60 düşme ile belirli) ve iki hastada kısmi bir düzelme (% 30 düzelme) belirlendi. Bir hasta Y- BOKÖ puanı çalışma öncesine göre anlamlı değişiklik göstermezken, bir hasta daha da kötüleşti. Bir hasta ise iştah artması ve aşırı sedasyon nedeniyle tedaviyi bıraktı. Tartışma: Sonuçlar tedaviye dirençli OKB'ta olanzapinin etkili ve iyi tolere edilen bir alternatif ekleme tedavisi olduğunu düşündürmektedir.

Objective: Although serotonin reuptake inhibitors (SRI) are used at the treatment of obsessive compulsive disorder (OCD) widely, almost 40-60 % of patients do not response to these medications. In our study, it was aimed to determine consequences of an atypical antipsychotic olanzapine augmentation in the patients with refractory OCD. Method: The sample consisted of 12 patients who had applied to Psychiatry Department of Fırat University Medical Faculty and diagnosed as OCD according to DSM- IV and used SRI alone or with another agent such as anxiolitic, mood stabilisator or neuroleptic at three months follow-up between October 1999- May 2000. The severity of OCD was evaluated by Yale-Brown Obsession Compulsion Scale (Y-BOCS) and the doses were increased in patients with insufficient response. Results: Seven of the patients exhibited significant (at least 60 % decrease) and partial improvement (30 % decrease) at Y-BOCS scores. While one patient had insufficient score changes, another patient became worse. One patient discontinued the treatment because of increasing body weight and excess sedation. Conclusion: Consequently, the results suggest that olanzapine is efficacious and tolerable alternative augmentation.
