Anksiyöz depresyon: Bir depresyon alt grubu mu?

Araştırmalar anksiyetenin klinik depresyonda en belirgin belirti olduğunu ve depresyonun başarılı sağaltımı için şiddetli ve dirençli anksiyetenin önemli olduğunu önermektedir. Anksiyöz depresyon terimi düşünülürken, hastaların anksiyete ve depresyon belirtilerini üç şekilde gösterebileceği göz önünde tutulmalıdır: Komorbid anksiyete ve depresyon, depresyon ve eşik altı anksiyete, eşik altı depresyon ve eşik altı anksiyete. Depresyon ve anksiyete arasındaki ilişkinin henüz net olmamasına rağmen anksiyöz depresyon teriminin eşik altı anksiyete belirtileri gösteren depresyon için kullanılması gerektiği belirtilmektedir. Depresyon hastalarında anksiyete, şiddet, düşük tedavi yanıtı ve özkıyımın belirtecidir. Yüksek anksiyeteye sahip depresyon hastalarında hastalık daha şiddetli, iyileşme daha uzun, antidepresanlara yanıt daha düşüktür. Bu yazıda bu grup hastalarda en uygun tedavi gözden geçirilmiştir. Bu konuda prevalans, klinik özellikler ve tedaviyi daha iyi anlamak için daha fazla çalışmaya gereksinim vardır.

Anxious depression: is it a subgroup of depression?

The current evidence suggests that anxiety is one of the most prevalent symptoms in clinical depression and that assessment and treatment of severe, persistent anxiety is of great importance in successful treatment of depression. When considering anxious depression term, it should be recognized that patients can manifest symptoms of depression and symptoms of anxiety in 3 separate ways; as comorbid depression and anxiety, as depression with subthreshold anxiety, and as subthreshold depression with subthreshold anxiety. The nature of the link between depression and anxiety remains uncertain, however, based on the evidence to date, the term anxious depression be reserved for patients who meet diagnostic criteria for depression and have subthreshold symptoms of an anxiety disorder. In depressed patients, anxiety is a marker of severity, poor outcome, poor response to treatment, and suicide risk. Depressed patients who have higher ratings for anxiety are more severely ill, take longer to recover, and show poor response to antidepressants. In this paper we aimed to review the most appropriate treatment for this group of patients. It is also evident that more research needs to be undertaken in this important patient group in order to understand its prevalence, clinical features, and treatment.
