Tolosa-Hunt sendromu ve diyabetik kraniyal mononöropati birlikteliği: Olgu sunumu ve gözden geçirme

Tolosa-Hunt sendromu (THS) ağrılı oftalmopleji ile prezente olan, steroide yanıtlı, kavernöz sinüs/superior orbital fissür’ün enflamasyonunun neden olduğu bir hastalıktır. Diyabetik hastalarda kraniyal nöropati nadir görülmekle birlikte yaşlı ve dü- zenli kan şekeri takibi yapılmayan hastalarda sık görülür. Elli altı yaşında kadın hasta periorbital ağrı ile eş zamanlı başlayan sağ VI., sol III., IV., VI., VII. kraniyal sinirler ve trigeminal sinirin maksiller dalı tutulumu ile başvurdu. THS ve diyabetik kraniyal nöropati birlikteliği sık görülmemekle birlikte eşzamanlı başlayan multipl kraniyal sinir felci ve ağrılı oftalmoplejisi olan hastalarda tanıda düşünülmelidir. Diyabetik hastamızı ilginç birliktelik ve THS’ye fasiyal sinir felcinin de eşlik etmesi nedeniyle tipik kraniyal maynetik rezonans (MR) görüntüleri ile sunmayı uygun bulduk.

The coexistence of Tolosa-Hunt syndrome and diabetic cranial mononeuropathy: a case report and a review of literatu

Tolosa-Hunt syndrome (THS) is described as painful ophtalmoplegia caused by nonspesi fic inflammation of cavernous sinus or su- perior orbital fissure, which is response to steroid theraphy. Cranial neuropathies in diabetic patients are extremel y rare and occur in older patients with poorl y controlled diabetes. A 56-year-old diabetic female patient who developed simultaneous right VI., left III., IV., VI., VII. and maxiller branch of trigeminal nerve palsies with acute periorbital pain applied to our outpatient clinic. Although the coexistence of THS and diabetic cranial neuropathy is an uncommon condition, this should be considered as a possible diagnosis on patients who are admitted with painful ophtalmoplegia and simultaneous multipl cranial neuropathies.
