Erişkin önkol diafizer çift kırıklarının cerrahi tedavisinde osteosentez materyali seçimi
Amaç: Erişkin önkol diafiz çift kırıklarının cerrahi tedavisinde en uygun osteosentez materyalinin hangisi olduğu araştırıldı. Çalışma planı: Cerrahi tedavi uygulanan 55 hasta (46 erkek, 9 kadın; ort. yaş 31.5) değerlendirildi. Kırıklar AO/ASIF’e göre sınıflandırıldığında, en sık A3 tip (%56.3) kırık saptandı. Kırıklarda plak vidayla osteosentez (%77.2), intramedüller osteosentez (%21.8) ve eksternal fiksatör (%1) uygulandı. Hastalar ortalama 22 ay (4 ay-9 yıl) süreyle takip edildi. Sonuçlar: Kaynama oranı radius için %94.5, ulna için %96.4 bulundu. Anderson kriterlerine göre değerlendirildiğinde 39 kırıkta mükemmel (%70.9), dokuz kırıkta iyi (%16.4), beş kırıkta yetersiz (%9.1) ve iki kırıkta kötü (%3.6) sonuç elde edildi. Çıkarımlar: Erişkin önkol diafizer çift kırıklarında kırık tipi, kemik kalitesi ve hastanın yaşı göz önüne alınmak suretiyle öncelikli olarak dinamik kompresyon plaklarının tercih edilmesi gerektiği kanaatindeyiz.
Selection of osteosynthesis material in the surgical treatment of adult forearm diaphyseal fractures

Ortopedi ve Travmatoloji Klinigi Objectives: This study sought to determine the most approapriate osteosynthesis material in the treatment of adult forearm fractures. Methods: A total of 55 patients (46 males, 9 females; mean age 31.5 years) underwent surgery for adult forearm fractures. According to AO/ASIF classification, most were type A3 fractures (56.3%). Treatment included plate-screw osteosynthesis (77.2%), intramedullary fixation (21.8%) and external fixation (1%). Mean follow-up was 22 months (range 4 months-9 years). Results: The union rates were 94.5% and 96.4% for radius ulna, respectively. According to Anderson's criteria, results were excellent in 39 cases (70.9%), satisfactory in nine cases (16.4%), unsatisfactory in five cases (9.1%), and poor in two cases (3.6%). Conclusion: The results showed that dynamic compression plates can be preferred initially in adult forearm diaphyseal fractures, with consideration given to the type of fracture, bone nature and the patient's age.
