Amaç: Akut anterior omuz ç›k›¤› nedeniyle kapal› redüksiyon sonras› d›fl ve iç rotasyonda tespit edilen hastalar›n fonksiyonel ve stabilite sonuçlar›n›n karfl›laflt›r›lmas›n› amaçlad›k. Çal›flma plan›: Primer travmatik anterior omuz ç›k›¤› tan›s› olan 33 hasta (31 erkek, 2 kad›n) iç rotasyonda (n=17) ve d›fl rotasyonda (n=16) tespit edildi. Hastalar› ortalama 20.85 ay (da¤›l›m 6- 41ay) takip ettik. Tespit sonras› hemen rehabilitasyon program›na bafllad›k. Takip süresi içinde 6. ayda, hastalar› fonksiyonel aç›dan Constant-Murlay skoru ile, stabilite aç›s›ndan ise Rowe skorlama sistemi ile de¤erlendirdik. Hastalar›n 12. ve 24. ay kontrolleri yap›ld›. Sonuçlar: ‹ç rotasyon ve d›fl rotasyon gruplar› aras›nda Constant-Murlay skoru ve Rowe skoru aç›s›ndan anlaml› fark saptanmam›flt›r. D›fl rotasyonda tespit edilenlerde %6.3 (1/16), iç rota- syonda tespit edilenlerde %29.4 (5/17) hastada tekrar omuz ç›k›¤› saptanm›flt›r (p>0.05). Fakat 21-30 yafl alt grubunda, d›fl rotasyonda tespit edilenlerde hiç tekrar omuz ç›k›¤› saptanmazken, iç rotasyon tespitinde 5 hastada tekrarlayan omuz ç›k›¤› saptanm›flt›r (p=0.035). Ç›kar›mlar: Akut anterior omuz ç›k›¤›nda redüksiyon sonras› d›fl rotasyonda tespit tekrarlayan ç›k›klar›n önlenmesi için etkili bir yöntemdir ve klinik uygulamada geleneksel iç rotasyonda tespitin yerine tercih edilmelidir.
