Amaç: Ortognatik cerrahide yaygın olarak kullanılan sagittal split osteotomi (SSO) tekniği ile mandibulanın saatin tersi yönünde rotasyonu ve ilerletilmesi sonrası posterior açılı kondil üzerindeki streslerin ‘Sonlu Eleman Analizi’ (SEA) metodu ile incelenmesi ve 5 farklı mandibuler ilerletme modeli oluşturularak kondilde meydana gelen stres alanlarının, stabilite ve relaps problemlerinin belirlenmesi amaçlanmıştır. Gereç ve Yöntemler: Çalışmamızda anterior açık kapanışı olan bir hastanın bilgisayarlı tomografi görüntüsü kullanılarak mandibula modeli oluşturulmuştur. Mandibula modeli üzerinde çift taraflı sagittal split osteotomi yapılarak 5 ve 10 mm ilerletme ve saatin tersi yönünde hareket işlemleri, ayrıca ilerletme sonrasında kemik segmentlerinin pasif ya da aktif fiksasyonu yapılarak 5 farklı kombinasyon oluşturulmuştur. Posterior açılı mandibuler kondil üzerine gelen baskı, gerilim ve Von Mises stresleri SEA metodu ile değerlendirilmiştir. Bulgular: Mandibuler kondilde stres yoğunluğu mandibulanın ilerletme miktarına ve fiksasyon tekniğine göre değişmektedir. İlerletme miktarı arttıkça kondilin posterioru ve lateralinde stres miktarı artmaktadır. Fiksasyon aşamasında kondilin posterior açılanması gelen stres miktarını ve yoğunluğunu arttırmaktadır. Mandibuler ilerletmeden sonra gonial açıda görülen değişiklikler Model 2 (124.420) > Model 4 (125.610) > Model 3 (128.350) > Model 5 (129.300) şeklindedir. Sonuç: Mandibuler ilerletme ve saatin tersi yönünde rotasyon yaptırılan olgularda, mandibuler ilerletme miktarı arttıkça kondilin üzerine gelen stres miktarı artmaktadır. Mandibuler kondilin fiksasyon sırasında orijinal pozisyonunu korunmaması ve posteriora açılanması ile üzerine gelen stres miktarı daha da artmaktadır. Bu durumlarda gözlenen stres artışı relapsa neden olabilmektedir.

Results: The stress intensity on the mandibular condyle varies according to the advancement amount of the mandible and the fixation technique. As the magnitude of advancement increases, the amount of stress on the posterior and lateral of the condyle increases. Posterior angulation of the condyle during fixation increases the magnitude and intensity of the stress. Changes in the gonial angle after mandibular advancement are as Model 2 (124.420)> Model 4 (125.610)> Model 3 (128.350)> Model 5 (129.300). Conclusion: In cases with mandibular advancement and counter-clockwise rotation, the amount of stress on the condyle increases as the amount of mandibular advancement increases.
