Hızlı Test ile İnfluenza A ve B Tanısının Klinik Bulgulara Göre Değerlendirilmesi

Amaç: İnfluenza virüslerinden kaynaklanan hastalıklar, hafif solunum yolu hastalıklarından ölümcül viral pnömoniye kadar çeşitlilik gösterir. Virüsün tanısı burun ve boğaz sürüntüleri, nazofaringeal aspirat, balgam ve bronkoalveolar lavaj gibi örneklerden konulabilinir. İnfluenzanın kesin tanısı virüsün hücre kültüründen izole edilmesi ile konulur. Ancak influenza epidemi ya da pandemisi varlığında hızlı tanı testleri viral hücre kültürü yöntemleri ile birlikte kullanılabilir. Materyal- Metot: Çalışmaya Mart-Nisan 2008 tarihleri arasında Zeynep Kamil Kadın Doğum ve Çocuk Hastalıkları Eğitim ve Araştırma Hastanesi Çocuk Acil Polikliniğine başvuran, şikayetleri son üç gün içerisinde başlayan, başka nedenle açıklanamayan ani ateş (>38°C), öksürük ve/ veya boğaz ağrısı olan 1 ay-14 yaşları arasındaki çocuk hastalar alındı Bulgular: Bu çalışmada, klinik olarak influenza düşünülen 100 hasta (47 kız ve 53 erkek) hızlı test ile değerlendirildi. Beş olguda test pozitif olarak değerlendirildi ve viral kültürde üreme saptanamadı. İnfluenza pozitif saptanan olgularda; ateş (% 100), öksürük (% 80), nazal akıntı (% 60) ve nazal obstrüksiyon (% 20) mevcut idi ve görülme oranları arasında istatistiksel olarak anlamlı farklılık yoktu (p>0,05). Miyalji ve başağrısı influenza pozitif olgularda, influenza negatif olanlara göre daha yüksek oranda görülmesine rağmen istatistiksel olarak anlamlı değildi. Sonuçlar: Hızlı tanı testlerinin kullanımının kolay ve İnfluenza tanısında kullanılabilir olmasına rağmen her zaman doğru sonuç alınamayabilir. Bu nedenle bu testlerin ancak tarama amaçlı olarak kullanılabileceği kanısındayız.

Evaluation of the Iimportance of Rapid Test and the Cinical Findings for the Diagnosis of Influenza A and B

Objective: Diseases caused by influenza viruses range from an upper respiratory tract infection to lethal viral pneumonia. The virus can be diagnosed by materials of nasal swaps, nasophargeal aspirates, sputum or bronchoalveolar lavage. The golden standart for diagnosis is the isolation of virus in cell cultures. During an epidemic or a pandemic, rapid tests and cultures can be used concomitantly. Material and methods: A retrospective review of vertical rectus muscle tIn this study, we aimed to evaluate the importance of rapid test and the clinical findings for the diagnosis of influenza in 100 children (47 girls and 53 boys) who were clinically compatible with influenza. Results: There were only 5 positive results whereas no virus was isolated with culture. When we compared the occurrence of fever (% 100), coughing (% 80), nasal drainage (% 60) and nasal occlusion (% 20) based on the rapid test positivity, the results were not statistically significant(p>0,05). Although myalgia and headache were more commonly seen in Influenza test positive patients, the results were not significant either. Conclusion: Although rapid tests are easy and appropriate to use for the diagnosis of influenza, they are not always reliable. For this reason, we believe that they can only be used for screening.
