İnmemiş testis: Erken tanı ve tedavinin önemi

Amaç: İnmemiş testis çocukluk çağında sık görülen konjenital anomalilerden biridir. Bu durum, ilerleyen yaşlarda testiste oluşabilecek komplikasyonlardan dolayı ilk bir yıl içerisinde tedavi edilmelidir. Biz inmemiş testis olgularımızı retrospektif olarak değerlendirdik.Gereç ve yöntem: Kliniğimizde Mart 1999- Haziran 2011 tarihleri arasında inmemiş testis nedeniyle tedavi edilen 219 olgu; yaşları, eşlik eden patolojiler, ailelerin eğitim durumu, komplikasyonlar ve tedavi açısından retrospektif olarak değerlendirildi.Bulgular: Yaşları 6 ay-31 yaş arasında değişen 64 bilateral, 86 sağ, 69 sol;toplam 219 olgu, 283 inmemiş testis incelendi. 235 inmemiş testise orşiopeksi uygulanırken, 48’ine orşiektomi uygulandı.Orşiektomi materyallerinin histopatolojik incelemesinde; 1 olguda seminom, 24 olguda sertoli cell only, 1 olguda leyding hücre hiperplazisi, 14 olguda tübüler atrofi, 6 olguda germinal aplazi, 2 olguda spermatogenetik arrest tespit edildi. Ek olarak 19 olgumuzda genitoüriner problemler, 11 olgumuzda diğer sistem problemleri mevcuttu. 5 olgumuz testis torsiyonu, 5 olgumuz inkarsere herniile bize başvurmuştu. Sonuç: İnmemiş testis tanı ve tedavisindeki gecikmelerden dolayı infertilite, testis kaybı ve malignite potansiyeli gibi komplikasyon oranları artmaktadır. Erken tedavi&düşük komplikasyon oranları için; aileler inmemiş testis ve sonuçları hakkında bilgilendirilmeli, sağlık personelinin konuya olan ilgisi arttırılmalıdır

Cryptorchidism: The importance of early diagnosis and treatment

Objective: Cryptorchidism is one of the congenital anomalies which are commonly seen during the childhood. It should be treated within the first one year because of the complications that will be able to occur in the testicle together with aging. We retrospectively evaluated our cases of cryptorchidism.Materials and Methods: Between March 1999 and June 2011, 219 patients whose treated because of cryptorchidism in our clinic were retrospectively evaluated in terms of the ages, concomitant pathologies,educational status of the families, complications and treatment.Results: Totally 219 of the cases whose ages ranged from 6 months to 31 years and 283 of undescended testicles whose 64 were bilateral, 86 were right and 69 were left were examined. While the orchiopexy was applied in 235 of undescended testicles, the orchiectomy was applied to 48 of undescended testicles. In the histopathologic examination of the orchiectomy materials, the seminoma in 1 case, the sertoli cell-only in 24 cases, the leyding cell hyperplasia 1 case, the tubular atrophy in 14 cases, the germinal aplasia in 6 cases, the spermatogenic arrest in 2 cases were detected. In addition, there were genito-urinary problems in 19 of our cases, and the other system problems in 11 of our cases. 5 of our cases with the testiculartorsion and the other 5 of our cases had applied to with the inkarsere hernia.Conclusion: The rates of the complications such as the infertility, the testicular loss and the potential for the malignancy due to the delays in the diagnosis and treatment of undescended testicles have increased. For early treatment&low rates of the complication as a result of this; families should be informed about the undescended testicle and its results and the interest of the health staff about the subject should be increased
