Bakteriyel Translokasyon

Bakteriyel translokasyon, yaklaşık yüz yıldır, canlı mikroorganizmaların intestinal epiteliyal bariyeri geçmesi olarak tanımlanmıştır. Bununla birlikte, mikrobiyal translokasyonun kesin mekanizması bilinmemektedir. Aktif epiteliyal hücrelerin bakterileri içine almaları ve fagositler aracılığı ile olan transport, mezenterik lenf yumruları ve uzak organlara bakterilerin ekstraintestinal hareketi için muhtemel etkenler olarak ileri sürülmüştür. Translokasyon, immunosupresif ve şiddetli hastalarda intestinal orijinli portal sepsisle birlikte asıl proses olarak ileri sürülmüştür. Oysa birçok etiyolojik faktör bakteriyel translokasyonun patojenitesinde etkili olabilir. Mevcut klinik gözlemler, yalnız köpek, fare ve rat çalışmalarına dayandırılmıştır. Halihazırda araştırmalar, gastrointestinal bakteriyel enfeksiyon ile intestinal mukozanın bütünlüğü arasındaki İlişkiyi ortaya koymak için devam etmektedir.

Bacterial Translocation

The phenomenon of bacterial translocation, the movement of viable indigenous microorganisms across the intestinal epithelial barrier, has been recognized for almost 100 years. At the present time, the precise mechanism of microbial transport is unknoum. Active ephitelial uptake and phagocyte-mediated transport have been proposed as likely eıvplanations for the extraintestinal movement of bacteria to mesenteric lymph nodes and distant organ sites. Translocation has been proposed as the pivotal process assosiated ıvith intestinal origin portal sepsis in seriously ili and immunocompromised patients. While several etiologic factors can be implicated in the pathogenesis of bacterial translocation, the present clinical implications are based almost solely on studies of dogs, rats and mice. Present investigations continue to suggest a causel relationship betıveen intestinal mucosal integrity and infection by indigenous gastrointestinal bacteria.
