Önceden sağlıklı olan bir çocukta ektima gangrenozum

Ektima gangrenozum, Pseudomonas aeruginosa enfeksiyonlarının iyi bilinen bir cilt bulgusudur ve genelde bağışıklığı baskılanmış hastalarda görülür. Öncesinde tamamen sağlıklı olan beş aylık bir erkek hasta açık yara yakınmasıyla başvurdu. Hastanın her iki kasık bölgesinde etrafı eritematöz bir hale ile çevrili ortası ülsere birkaç adet lezyon gözlendi. Lezyondan alınan sürüntü kültüründe Pseudomonas aeruginosa üretildi. Kan kültüründe üreme olmadı. Hastaya bakteriyemik olmayan ektima gangrenozum tanısı konuldu. Hastanın immünolojik değerlendirilmesi normal bulundu. Anti- pseudomonal antibiyotik olan sefaperazon-sulbaktam ile 10 gün boyunca tedavi edildi. (Türk Ped Arfl 2013; 48: 68-70)

Ecythema gangrenosum in a previously healthy child

Ecthyma gangrenosum is a well known cutaneous manifestation of Pseudomonas aeruginosa infections and is usually seen in immunocompromised patients. A previously healthy 5-month-old boy presented to our hospital with a complaint of an open inguinal wound. Physical examination revealed several ulcers surrounded by erythematous halos in bilateral inguinal regions. Pseudomonas aeruginosa was isolated from swab cultures obtained from the ulcers. However, blood cultures were sterile. The patient was diagnosed as non-bacterial ectima gangrenosoum. No immunological abnormality was detected during extensive investigation of the patient. The patient was successfully treated with an anti-pseudomonal antibiotic cefoperazone-sulbactam. (Turk Arch Ped 2013; 48: 68-70)
