Çocukluk çağı tüberküloz enfeksiyonunun farklı klinik tablolarında T lenfosit ve alt gruplarının sayısal değerlerini saptamak elde edilen veriler ile tedavi PPD yanıtı ve hastalığın klinik tablosu arasında ilişki olup olmadığını göstermek amacı ile 26 tüberkülozlu hasta çalışma kapsamına alındı Kontrol grubu 14 sağlıklı çocuktan oluşturuldu nbsp; İlk tanı konulduğunda saptanan T4 lenfosit sayılan kontrol grubu ile karşılaştırıldığında anlamlı olarak düşük bulunurken T ve T8 lenfosit sayılarında anlamlı fark tespit edilmedi T4 T8 oranı 2 hastada 1 in altında 9 hastada 1 6 nın altında idi Tedavinin 8 haftasında hasta grubu T T4 ve T8 lenfosit sayıları ile kontrol grubu sonuçları ve hastaların tedavi öncesi sonuçları arasında anlamlı fark bulunmadı Tedavi öncesi T4 T8 oranı ters dönmüş olan 2 hastada bu oran 8 haftada 1 in üstüne çıktı Başlangıçta T4 T8 oranı 1 6 nın altında olan 9 hastanın yedisinde ise 1 6 nın üzerinde bulundu Hastalar reaktif olmayan ve reaktif grup olarak ikiye ayrıldığında reaktif olmayan grubun lenfosit T T4 ve T8 lenfosit sayıları anlamlı olarak düşük bulundu PPD yanıtı ile T T4 ve T8 lenfosit sayıları arasında anlamlı ilişki bulunamadı nbsp; Bu sonuçlar ışığında tüberküloz enfeksiyonunda saptanan T lenfosit ve alt grup değişikliklerinin bu hücrelerin aktif lezyon alanlarına göç etmesinden kaynaklanabileceği düşünüldü Anahtar kelimeler: Tüberküloz T Lenfopeni T4 Lenfopeni

Peripheral blood T lymphocytes and their subsets were determined in 26 patients with different clinical types of tuberculosis and the relationships between the number of T lymphocytes and treatment PPD reaction and clinical situation were discussed Fourteen healthy children were studied as the control group nbsp; In these patients the number of T4 lymphocytes were found to be significantly decreased when compared with those of the control group On the other hand the numbers of T and T8 lymphocytes were similar in the patients and the controls The T4:T8 ratio was disturbed in 11 patients: It was less than 1 6 in 9 patients Following 8 weeks of chemotherapy there was neither any significant difference between the posttreament findings and the original values nor between the posttreatment findings and the control values The reversed T4:T8 ratio in the 2 patients became greater than 1 in the 8th week of chemotherapy In 7 of 9 patients in whom the T4:T8 ratio was less than 1 6 the ratio became greater than 1 6 with chemotherapy When patients were separated into 2 groups as unreactive and reactive tuberculosis in the unreactive group significant decreases was found in lymphocytes T lymphocytes and their subsets There was no correlation between the number of T cells and their subsets and the magnitude of the delayed type hypersensitivity which was investigated by the tuberculin reaction PPD nbsp; T4 lymphopenia in tuberculosis is probably due to the local recruitment of T4 cells at sites of granuloma formation Key words: Tuberculosis T Lymphopenia T4 Lymphopenia
