B12 vitamini eksikliğine bağlı West sendromu

B12 vitamini bedende çeşitli sistemleri etkileyen esansiyel vitaminlerden biridir. B12 vitamini eksikliği süt çocuklarında sıklıkla makrositer anemi, nörogelişimsel gecikme ya da gerileme, irritabilite, güçsüzlük, hipotoni, ataksi, apati, tremor ve nöbetleri kapsayan hematolojik ve nörolojik bozukluklara neden olmaktadır. Biz bu makalede B12 vitamini eksikliği ile ilişkili West sendromu tanısı alan altı aylık erkek olguyu sunduk. Tam kan sayımında makrositer anemi ile uyumlu bulgusu olmayan hastanın hipotonik olması nedeniyle bakılan B12 vitamini düzeyi düşük olarak saptandı. West sendromu etiyolojisine yönelik diğer incelemelerde başka sorun saptanmadı. Olgu sadece anne sütü ile besleniyordu ve B12 vitamini eksikliğinin nedeni annenin beslenme yetersizliği idi. Farklı nörolojik bulgularla gelen hastalarda B12 vitamini eksikliği ayırıcı tanıda düşünülmelidir. Ayrıca etiyolojisinde çok farklı etmenlerin rol oynadığı West sendromunda nadir bir neden olarak B12 vitamini eksikliğinin olabileceği göz önünde bulundurulmalıdır

West syndrome due to vitamin B12 deficiency

Vitamin B12 is one of the essential vitamins affecting various systems of the body. Vitamin B12 deficiency in infants often produces haematological and neurological deficits including macrocyticanaemia, neurodevelopmental delay or regression, irritability, weakness, hypotonia, ataxia, apathy, tremor andseizures. In this article, we report the case of a 6-month-old male patient diagnosed with West syndrome associated with vitamin B12 deficiency. Although the patient had no evidence of macrocytic anemia in complete blood count, we measured the level of vitamin B12 because the patient had hypotonicity and found it to be low. No other problem was found in the other investigations directed to the etiology of West syndrome. He was being exclusively breastfed and vitamin B12 deficiency was related with nutritional inadequacy of his mother. Vitamin B12 deficiency should be considered in the differential diagnosis of patients presenting with different neurological findings. Inaddition, vitamin B12 deficiency should be considered as a rare cause in West syndrome which has a heterogeneous etiology
