Giresun ilinden hafif seyirli bir hantavirüs olgusu; olgu sunumu
Ülkemizde son birkaç yıldır gündeme gelen hantavirüs enfeksiyonları kronik olarak enfekte olan rodent çıkartılarının inhalasyonu ile nadiren rodent ısırığı yoluyla bulaşan zoonotik enfeksiyonlardır. Avrupada rodent popülasyonundaki değişikliklere bağlı olarak aralıklı salgınlarla artan sayılarda olmak üzere endemik görülen bölgeler mevcuttur. Ülkemizde ise 2009 yılında Zonguldak-Bartın bölgesinde 12si serolojik olarak doğrulanmış 25 vakanın görüldüğü ilk salgın ardından sporadik olgular bildirilmektedir. Bu raporda başlangıçta renal yetmezliği olmayan, ateş yüksekliği ve gastroenterit tablosu ile başvurup hafif trombositopeni saptanan bir vaka sunulmuştur. 45 yaşında bayan hastada kuru öksürük olması ve 2009 influenza salgını sırasında başvurması nedeniyle önce H1N1 influenzadan şüphelenilmiş, ardından sonucun olumsuz olması ve renal yetmezlik eklenmesi sonrasında hantavirüs enfeksiyonu düşünülmüştür. Refik Saydam Hıfzıssıhha Merkezi (RSHM) Viroloji Referans ve Araştırma Laboratuvarına gonderilen serum örneğinde İndirekt immunfloresans assay (IFA) yöntemi (Hantavirus Mosaic-1; Euroimmun, Almanya) ile hantavirus Dobrava IgM ve IgG antikorları ≥ 1: 100 titrede pozitif bulunmuş ve bu sonuç immunoblot (IB) yöntemiyle de (Hantavirus Profile 1 EUROLINE IgG ve IgM; Euroimmun, Almanya) pozitif olarak görülmüştür. Olgumuzda her iki yöntem ile de Dobrava serotipine karşı seropozitiflik saptanmıştır. Bu vaka dolayısıyla özellikle Karadeniz Bölgesindeki klinisyenlerin renal fonksiyon bozukluğu olmasa da ateş ve trombositopeni ile başvuran hastalarda Hantavirüs enfeksiyonunu da akla getirmesi gerektiğine dikkat çekilmek istenmiştir.
Hantavirus infection with a mild course in a patient from Giresun province: A case report
Hantavirus infections, which became a current issue last few years in our country are zoonotic infections that are transmitted through aerosols from chronically infected rodents excreta, and occasionally rodent bites. In Europe there are endemic regions that the disease is seen with increasing prevalence and intermittent outbreaks due to variations in rodent population. In our country sporadic cases are reported following the first outbreak in Zonguldak-Bartın region consisting of 25 cases, of whom 12 were serologically identified as Pumaala subtype in 2009. In this report a case that admitted to our clinic with fever and gastroenteritis, having mild thrombocytopenia but not renal involvement at admission. Forty-five years old women had been suspected of having H1N1 influenza as she had a dry cough, and her admission was during 2009 influenza outbreak, but after H1N1 tests were found to be negative and renal failure developed, hantavirus infection came into consideration. Patient s serum sent to National Reference Virology Laboratory by IFA (Hantavirus Mosaic-1, Euroimmun, Germany) revealed hantavirus IgM and IgG antibodies ≥ 1: 100 titer and the results were also positive by immunoblot test (Hantavirus Profile 1 EUROLINE IgG and IgM, Euroimmun, Germany). In our patient, Hantavirus Dobrava subtype seropositivity was determined by two tests. With this report we aim to tell clinicians especially working in the Black sea region to take into consideration hantavirus infections for the patients admitting to hospitals with fever and thrombocytopenia, even if they do not suffer renal insufficiency.
