Plevranın soliter fibröz tümörü:

Soliter fibröz tümör plevranın nadir bir patolojisidir ve sıklıkla visseral plevradan köken alır. Genellikle selimdir, fakat olguların %10-20’si malign bir seyre sahip olabilir. Klinik bulgular genellikle tümörün büyüklüğüne ve yerleşim yerine göre değişmektedir. Hastalığın erken döneminde olguların çoğu asemptomatiktir. Bununla birlikte tümör büyüdükçe öksürük, göğüs ağrısı ve nefes darlığı gibi solunum semptomları ve hipoglisemi ve çomak parmak gibi ekstrapulmoner belirtiler görülebilmektedir. Kesin tanı için genellikle kitlenin torakotomi aracılığı ile total olarak çıkarılması gerekmektedir. Tümörün cerrahi olarak çıkarılması tedavi için yeterli olsa da, ameliyat sonrası küçük bir nüks riski vardır. Histolojik olarak tümör, fibroblast benzeri hücrelerden ve bağ dokusu hücrelerinden oluşmaktadır. Tümör hücreleri genellikle CD34 ile pozitif boyanır. Bu makalede kliniğimizde sağ hemitoraksta kitle nedeniyle ameliyat edilen ve ameliyat sonrası tanısı plevranın soliter fibröz tümörü olarak bildirilen olgunun klinik bulguları ve radyolojik, histolojik ve immünhistokimyasal özellikleri literatür eşliğinde tartışıldı.

Solitary fibrous tumor of the pleura:

Solitary fibrous tumor is a rare pathology of the pleura that frequently arises from the visceral pleura. It is usually a benign tumor, but 10-20% of the cases may have a malign course. Clinical findings usually vary with the size and the location of the tumor. In the early stages of the disease, most of the cases are asymptomatic. However, as the tumor enlarges, respiratory symptoms including cough, chest pain, and dyspnea, as well as extrapulmonary signs such as hypoglycemia and clubbing can be seen. To make a definitive diagnosis, generally the total excision of the mass through a thoracotomy should be performed. Although surgical resection is adequate for the treatment, there is a small risk of recurrence after surgery. The tumor histologically consists of fibroblast-like and connective tissue cells. The tumor cells usually stain positively with CD34. In this article, the clinical findings and the radiological, histological, and immunohistochemical characteristics of a patient whose diagnosis was reported as the solitary fibrous tumor of the pleura after being operated on for a mass in the right hemothorax are discussed in view of the literature.
