Amaç: Bu çalışmada genç erişkin yaş grubundaki pektus ekskavatumlu hastalarda yapılan Nuss işlemi sonuçları değerlendirildi.Çalışma planı: Mart 2010 - Aralık 2013 tarihleri arasında kliniğimizde Nuss işlemi ile ameliyat edilen 23 erkek hasta (ort. yaş 21.7 yıl; dağılım 21-26 yıl) geriye dönük olarak değerlendirildi. Hastaların klinik verileri, ameliyatın özellikleri ve komplikasyonlar kaydedildi. Hastalara ameliyat sonrası ortalama 28.7 ayda (dağılım 3-45 ay) tek basamaklı bir anket uygulandı. Hastaların Nuss işlemi sonrası genel memnuniyet durumu değerlendirildi.Bul gu lar: Göğüs şeklinin ameliyat öncesi sosyal aktiviteye negatif etkisi %95.7 iken, ameliyat sonrası bu değer %30'a geriledi. Hastaların %66.6'sının hastanede kaldığı dönemde orta veya şiddetli ağrı duyduğu, %83.3'ünde taburcu olurken hafif ağrı olduğu, %77.7'sinde ise ameliyat sonrası üçüncü ayın sonunda ağrının kaybolduğu tespit edildi. Özgüven ortalaması değeri ameliyat öncesi 5.2 iken, ameliyat sonrası bu değer 7.4'e yükseldi (Wilcoxon işaretli sıra testi, p<0.05). Hastaların %88.8'i sonuçtan memnun veya çok memnun olduğunu ifade etti. On dört hastada komplikasyon (%77.7) görüldü. Beş hastada bar kayması (%27.7), sekiz hastada pnömotoraks (%44.4) (sadece iki tanesi tüp torakostomi gerektiren), bir hastada plevral efüzyon (%4.3), bir hastada ısrarcı ağrı (%4.3) ve bir hastada yara yeri sorunu (%4.3) saptandı.So nuç: Memnuniyet ve komplikasyon oranı dikkate alındığında, Nuss işleminin genç erişkin hastalarda güvenle kullanılabileceğini ve tercih edilmesi gereken yöntem olduğunu düşünüyoruz. Daha büyük örneklemli ve takip süresi uzun çalışmalar bu konuya ek açıklık getirebilir.

Patients' clinical data, features of the operation, and complications were recorded. A single stage questionnaire was applied to patients at a mean of 28.7 months (range 3-45 months) after the operation. Overall satisfaction status of patients after the Nuss procedure was evaluated.Results: While the negative effect of chest shape to social activity was 95.7% preoperatively, this value decreased to 30% postoperatively. It was detected that 66.6% of patients suffered from moderate or severe pain during hospital stay, 83.3% had mild pain during discharge, and pain disappeared in 77.7% at the end of the postoperative third month. While the mean self confidence value was 5.2 preoperatively, this value increased to 7.4 postoperatively (Wilcoxon signed-rank test, p<0.05). Of the patients, 88.8% expressed that they were satisfied or highly satisfied with the result. Complications were observed in 14 patients (77.7%). Bar shift was detected in five patients (27.7%), pneumothorax in eight patients (44.4%) (only two of them required tube thoracostomy), pleural effusion in one patient (4.3%), persistent pain in one patient (4.3%), and wound site problem in one patient (4.3%).Conclusion: Considering the satisfaction and complication rate, we think that the Nuss procedure can be safely used in young adult patients and should be the preferred method. Studies with larger sample size and long-term follow-up may provide additional clarification to this issue. ">
