Amaç: İntrakoroner stent implantasyonu sonrası görülen restenoz veya yeni gelişen lezyonlar yeniden girişimleri zorunlu kılmaktadır. Çalışmamızın amacı geç dönem takipte koroner arter bypass cerrahisi (KABC) gereken olgular ile, cerrahi dışı tedavi uygulanan olguların karşılaştırılması ve KABG için prediktif özelliklerin araştırılmasıdır. Materyal ve Metod: 1999-2002 tarihleri arasında kliniğimizde koroner stent uygulanıp, geç dönem koroner anjiyografisi sonrası KABG kararı alınan 73 olgu (A grubu; 61/E, yaş ort. 54.1 ± 11) ile kontrol anjiyografisi sonrası medikal veya invaziv takip edilen 84 ardışık olgu (B grubu 71/E, yaş ort. 52.7 ± 12) çalışmaya alındı. Her iki grup arasında stent uygulandığı dönemdepreoperatif risk faktörleri bakımından farklılık yoktu. Lezyon anatomisi açısından bakıldığında ise tip C lezyon, A grubunda (p = 0.003), tip B ise B grubunda (p = 0.01) daha fazlaydı.Bulgular: Stent implantasyonu sonrası A grubunda sigara kullanımı %11, B grubunda ise %41 azalma göstermişti (p < 0.001). A grubunda dislipidemi kontrolü olguların %5’inde, B grubunda %27 olguda (p < 0.001) sağlanmıştı. Hiperglisemi A grubundaki diyabetik olguların %67’sinde, B grubunda ise tamamında kontrol altına alınmıştı (p = 0.009). Kontrolsüz hipertansif hasta A grubunda %5, B grubunda %13 oranında azalma göstermişti (p < 0.0001). Multivariate analizde stent sonrası hiperlipidemi olması, sigara kullanımı ve C tipi lezyon operasyona gidişi anlamlı olarak etkileyen parametreler olarak belirlendi. Koroner arter bypass cerrahisi grubunda operasyonda %78 oranında hedef lezyon re-revaskülarizasyonu, ve %90 oranında hedef lezyon dışı revaskülarizasyon gerçekleştirilirken bu oranlar B gurbunda sırası ile %12 ve %14 idi.Sonuç: Stent implantasyonu sonrası sigara kullanımı, hiperlipidemi ve C tipi lezyon olması hastaların cerrahiye konversiyonunda rol oynayan faktörlerdendir.

The aim of this study was to compare the patients who underwent coronary artery bypass graft operation or who were under follow up with medical therapy after stent implantation, and to investigate the risk factors for surgical reintervention. Methods: Between 1999 and 2002, 157 patients who received stent implantation were included in this study. Seventy-three patients who underwent CABG operation after a control coronary angiography with a mean age of 54.1 ± 11 formed the group A, and 84 patients who were under follow up with medical therapy after a control coronary angiography with a mean age of 52.7 ± 12 formed the Group B. There were no differences in the terms of age, sex, hypertension, type II diabetes mellitus and smoking incidences between the two groups. Type C lesion were more common in Group A than in Group B (p = 0.003).Results: After stent implantation, the smoking incidence reduced a 11% in group A and 41% in Group B (p < 0.001). Dyslipidemia was controlled in 5% of the patients in group A and 27% of Group B (p < 0.001). Hyperglycemia were seen in 67% of the diabetic patients in Group A. All of the diabetic patients in Group B were normoglycemic in this period. The decreased rate of uncontrolled hypertension was 5 % in Group A and 13% in Group B. In multivariate analysis, hyperlipidemia, smoking and type C lesion were the predisposing factors of CABG operation after stent implantation.
