Akciğerin sekonder tümörlerinde metastazektomi sonuçları: 74 olgunun retrospektif değerlendirilmesi

Amaç: Bu çalışmada, pulmoner metastazektominin sağkalıma olan katkısı ve uzun dönem sağkalıma etki eden prognostik faktörler araştırıldı.Çalışma planı: Ocak 1994-Aralık 2003 tarihleri arasında, pulmoner metastazektomi uygulanan 74 hasta (42 erkek, 32 kadın; ort. yaş 34; dağılım 3-73) çalışmaya alındı ve retrospektif olarak incelendi. Hastalar yaş, cinsiyet, primer tümör histolojisi, lezyon sayısı, ameliyat teknikleri, komplet ve inkomplet rezeksiyon sayıları, mediasten lenf nodu tutulumu, hastalıksız yaşam süresi, ek tedavi yönünden klasifiye edildi. Bu prognostik faktörlerin iki ve beş yıllık sağkalım üzerine etkileri değerlendirildi.Bulgular: Hastaların 12'sinde (%16) semptom bulunurken, geri kalan 62 (%84) hasta asemptomatikti. Olguların 41'inde (%56) sarkom metastazı, 20'sinde (%26) epitelyal tip tümör saptandı. Yetmiş dört hastanın 70'ine komplet metastazektomi, dördüne ise sadece tanısal amaçlı inkomplet rezeksiyon uygulandı. Ortalama ameliyat sonrası hastanede kalış süresi 5.7 (4-8 gün) gün, komplikasyon oranı %16 idi. Ortalama hastalıksız yaşam süreleri; sarkomlarda 16.2 ay, epitelyal tümörlerde 36 ay, germ hücreli tümörlerde 7.2 ay, melanomlarda 8.4 ay, diğer tümörlerde ise 35.5 ay olarak tespit edildi.Sonuç: Pulmoner metastazların cerrahi rezeksiyonu güvenli ve efektif bir tedavi şeklidir. Tümörün histopatolojisi, hastalıksız yaşam süresi, metastaz sayısı, mediastinal lenf nodu tutulumu ve ek tedavi önemli prognostik göstergelerdir. Metastaz sayısının surviye olan etkisinin net olarak belirlenebilmesi için daha geniş çalışmalara ihtiyaç vardır.

Outcomes of metastasectomy in secondary tumors of the lung: A retrospective analysis of 74 patients

Background: In this study, we examined the contribution of the pulmonary metastasectomy to survival and prognostic factors affecting long term survival.Methods: A total of 74 patients (42 males, 32 females; mean age 34 years; range 3 to 73 years) who underwent pulmonary metastasectomy between January 1994 and December 2003 were retrospectively reviewed. Patients were classified according to age, gender, primary histology of tumor, the number of lesions, operative techniques, the number of complete or incomplete resections, mediastinal lymph node involvement, disease free interval and additional therapy. Effects of these prognostic factors to 2 and 5 year-survival were evaluated.Results: Twelve of the patients (16%) were symptomatic whereas 62 patients (84%) were asymptomatic. Of the 74 patients, metastasis of sarcoma was seen in 41 (56%) and epitelial type tumor in 20 (26%) patients. Complete metastasectomy was performed in 70 patients whereas 4 patients underwent incomplete resection for diagnostic purposes. Mean postoperative hospital stay was 5.7 (4-8 days) and postoperative complication rate was 16%. Mean disease free interval was 16.2 months for sarcomas, 36 months for epitelial tumors, 7.2 months for germ cell tumors, 8.4 months for melanomas and 35.5 months for the other tumors.Conclusion: Surgical resection of pulmonary metastases is an effective and safe treatment modality. The histopathology of the tumor, disease-free interval, number of metastases, involvement of mediastinum and additional therapy are important prognostic markers. Larger series are needed to find out the clear contribution of the number of metastases to survival.
