Hipotiroidizmde solunum merkezinin karbondioksit ile uyarılabilirliğinin ölçülmesi
Bu çalışma, hipotiroid hastalarda gözlenen hipoventilasyonun sıklığını ve tedaviyle geri dönüşümünü değerlendirmek amacıyla yapılmıştır. Yirmisekiz hipotiroid hastanın oluşturduğu çalışma grubunda ve yaş-cinsiyet olarak uyumlu 18 sağlıklı bireyin oluşturduğu kontrol grubunda; Read DJ yöntemi modifiye edilerek elde edilen yeni bir yöntemle, solunum merkezinin hiperkapniye cevabı belirlendi. Hipotiroidik hastalardan 10 tanesinin hiperkapniye solunum merkezi cevabı (HSMC) değerleri, sağlıklı kontrol grubun en düşük değerinden daha düşük bulundu. Diğer yandan, hipotiroidik hastaların ortalama HSMC değeri, kontrol grubun ortalama HSMC değerinden anlamlı oranda düşüktü (1.01 ± 0.3 ve 1.26 ± 0.24 L/dak/mmHg, p< 0.05). Oniki haftalık tiroid hormon replasman tedavisiyle hastalar ötiroid olduktan sonra, bu değerler önemli ölçüde arttı ve sağlıklı kontrol grubuyla aynı düzeye ulaştı (1.0 ± 10.3 ve 1.28 ± 0.22 L/dak/mmHg, p< 0.001). Hipotiroidik hastaların serum TSH düzeyleriyle HSMC arasında anlamlı negatif korelasyon vardı (r= -0.84, p< 0.05). Hipotiroidik kadın hastaların HSMC değerleri düşük olmaya eğilimli olmalarına rağmen, farklılık istatistiksel olarak anlamlı değildi. HSMC değerleri ile diğer parametreler arasında belirgin bir bağlantı yoktu. Sonuç olarak, yüksek TSH düzeyi olan hipotiroidik hastalarda hiperkapniye solunum cevabının azalabileceği, bunun basit ve ucuz bir yöntemle belirlenip, tiroid hormon replasmanıyla solunum cevabındaki azalmanın geri dönebileceğini saptadık.
Measurement of ventilatory responsiveness in hypothyroidism
Hypothyroidism is known to be associated with abnormalities in both pulmonary function and ventilatory control. The aim of this study is to evaluate the frequency and reversal of hypoventilation seen in many of hypothyroid patients. In our study group, we modified Read DJ method to evaluate the ventilatory response of respiratory center to hypercapnia in 28 patients with hypothyroidism and 18 healty control subjects with the same age and sex. The ventilatory response to hypercapnia in 10 of 28 patients with hypothyroidism were found lower than the least value of the healthy control group. On the other hand, the mean value of ventilatory response to hypercapnia in patients withn hypothyroidism were significantly lower than that of control group (1.01 ± 0.3 and 1.26 ± 0.24 L/min/mmHg, p< 0.05). When the patients got euthyroid, after 12 weeks of thyroid hormone replacement therapy, these values increased significantly and reached the level similar to the healthy control group (1.01 ± 0.3 and 1.28 ± 0.22 L/min/mmHg, p< 0.001). There was a significant negative correlation between serum TSH levels and ventilatory response to hypercapnia in hypothyroid patients (r= -0.84, p< 0.05). Although it was assessed that the ventilatory response to hypercapnia in female patients with hypothyroidism tend to become lower, this was not statistically significant, there was no significant relation between ventilatory response to hypercapnia and the other parameters. As a result, we conclude that, the ventilatory response to hypercapnia in hypothyroid patients with high levels of TSH might be blunted, this may be assessed with a practical and bettr cost effective method, and thyroid hormone replacement therapy can reverse the blunting of ventilatory responsiveness.
