Elazığ Ruh ve Sinir Hastalıkları Hastanesi hasta ve çalışanlarında tüberkülin deri testi sonuçlarında booster etkisi
1-15 Eylül 1996 tarihleri arasında Elazığ Ruh ve Sinir Hastalıkları Hastanesi'ne yatırılarak izlenen hastalarda ve hastane çalışanlarında, tüberkülin deri testi sonuçlarına booster etkisini araştırmak amacıyla tüberkülin testi uygulandı. Çalışma grubu, yaşları 20-78 (ortalama 35 ± 12) arasında değişen, 265'i (%67.4) yatan hasta ve 128'i (%32.5) hastane çalışanı olmak üzere toplam 393 kişiden oluşturuldu. Her bireye 0.1 ml PPD (5 TÜ, İnterVax Biologicals Limited, Canada) uygulandı. 72 saat sonra uygulama yerinde oluşan endurasyon çapı ölçüldü. PPD testi negatif olanlardan 80 olguya, 7 gün sonra ikinci PPD testi uygulandı. Olguların %12.5'inde booster etkisi saptandı. Yaş grupları arasında veya önceden BCG aşısı uygulanmış ve uygulanmamış gruplar arasında booster etkisi görülme sıklığı yönünden istatistiksel olarak önemli düzeyde fark saptanmadı (p> 0.001). Booster etkisini araştırdığımız 80 olgunun 64'ü (%80) hasta grubunda, 16'sı (%20) hastane çalışanı grubunda bulunuyordu. Booster etkisi saptadığımız olguların tümü hasta grubunda idi. Sonuç olarak tüberkülozla ilgili epidemiyolojik çalışmalarda, PPD konversiyonlarına dayanılarak tanı konmasında ve koruma tedavisine karar verilmesi aşamasında, booster etkisinin araştırılmasının daha sağlıklı kararlara ulaşmada yararlı olacağını düşünmekteyiz.
The booster effect on the results of tuberculin skin test in patients and health workers in Elazığ Psychiatry Hospital
Tuberculin skin test was applied to the hospitalized patients and the staff of Elazığ Psychiatry Hospital from 1st to 15th September, 1996 with the intention of investigating booster effect on tuberculin skin test. The study group was 393 individuals aged between 20 and 78 (mean 35 ± 12). 265 individuals (67.4%) were patients and 128 individuals (32.5%) were hospital staff. To all subjects, 0.1 ml of PPD (5 TU, InterVax Biologicals Ltd, Canada) was applied. Diameter of enduration was measured after 72 hours. After 7 days, the second PPD test was applied in 80 cases, who had negative PPD results. The booster effect was detected in 12.5% of the subjects. There was statistically no significant difference between various age groups and between previously BCG administered group and group who did not receive BCG previously (p> 0.05). The group of 80 individuals, in whom the booster effect was investigated, included 64 patients (80%) and 16 hospital staffs (20%). All the subjects presented the booster effect were in the patient group. In conclusion, we suggest that it may be a useful tool to investigate the booster effect in reaching to appropriate decision in detecting PPD conversions, for prophylactic treatment and for epidemiological studies.
