Dizel egzosunun kronik solunumsal etkileri
Bir kuruluşun otobüs garajlarında çalışan 351 Dizel Egzosuna (DE) maruz işçi ve benzer yaş, boy dağılımı gösteren 274 kontrol ile çalışıldı. Olguların semptomları anket ile öğrenilerek fizik incelemeleri yapıldı, !ş yerinde fotofluografüeri çekildi ne SFT uygulandı. DE maruz işçiler ne kontroller arasında solunum semptomları ue kronik bronşit sıklığı yönünden fark bulunmadı. Sigara içen DE maruz işçiler ile sigara içen kontroller arasında kronik bronşit sıklığı yönünden fark bulunma-dı. Sigara içen DE maruz işçilerdeki kronik bronşit sıklığı, sigara içmeyen ueya sigarayı bırakmış işçilerdeki kronik bron-şit sıklığından anlamlı olarak daha fazla bulundu. Çalışma sürelerine göre oluşturulan DE maruz gruplardaki kronik bron-şit sıklığı, çalışma yılı artışı ile artış gösterdi. SFT'leri yönünden DE maruz işçiler ue kontroller arasında anlamlı farklılık bu-lunmadı. Sigara içen, içmeyen ue bırakmış DE maruz işçiler ile kontroller arasında SFT parametreleri yönünden fark bu-lunmadı. Çalışma yıllarına göre oluşturulan DE maruz gruplarda; çalışma yılı artışı ile SFTparametrelerinde azalma göste-rilemedi. DE maruziyeüne bağlı semptom - kronik bronşit sıklığında artış ve SFT parametrelerinde bozukluk gösterilemedi. Bu nedenle DE maruziyeti olanlarda kronik haua yolu obstrüksiyonu oluşabileceği düşünülmedi. DE'nun kronik akciğer hastalığı için yine de riskli maruziyet olarak kabul edilmesi ue bu konuda kesin bir konsensüs oluşuncaya kadar uzun sü-reli çalışmaların yapılması gerektiği sonucuna uarılmıştır.
Chronic respiratory effects of diesel exhaust
We conducted a study on workers who worked in the official bus garage in Ankara. 351 of them were exposed to Diesel Exhaust (DE). We also selected 274 controls from the same work place. Both groups were matched by age and height Symptoms of workers were assessed by a questionnaire and pysical examinations were done. Photofluoragraphies were obtained and Pulmonary function tests (PFT) was applied at work place. There was no difference between workers and controls with respect to respiratory symptoms and frequency of chronic bronchitis. Also no difference was obserued betwe-en the workers and control groups who smoked. Frequency of chronic bronchitis in smoker DE exposed workers was fo-und higher than ex-smoker or nonsmoker workers. Frequency of chronic bronchitis was increased as total working period increased when DE exposed workers were grouped according to working years. PFTparameters showed no significant dif-ference in DE exposed workers and controls. There was no difference in PFT parameters among smoker, nonsmoker and ex-smoker DE exposed workers. In DE exposed workers grouped by working years; there was no decrease in PFT parameters with an increase in working years. There was no increase in chronic bronchitis frequency with DE exposure related symp-toms and pulmonary function deterioration. In conclusion, chronic airway obstruction is not likely to be thought to deve-lop with DE exposure. Nonetheless, DE exposure should be considered to carry a risk with respect to chronic lung disease and require further studies until a final consensus is reached.
