Diş teknisyeni pnömokonyozu olgusu
Diş teknisyenlerinde silikozis, ekstrensek allerjik alveolit (EAA), gibi solunumsal problemler gelişebileceği 1939’lardan beri bilinmektedir. Diş teknisyenlerinde karmaşık içerikli maddelerin neden olduğu interstisyel akciğer hastalığı, diş teknisyeni pnömokonyozu olarak özel bir grupta incelenir. 36 yaşında, hiç sigara öyküsü bulunmayan olgunun ağır nefes darlığı şikayeti mevcuttu. 22 yıl diş atölyesinde döküm ve kumlama işi yapan olguda 5 yıl öncesine kadar solunumsal yakınma öyküsü bulunmamaktaydı. Bu tarihten itibaren çeşitli sağlık kuruluşlarında akciğer patolojisi açısından tetkik edilen ve yatarak tedavi gören olgu, solunum sıkıntısının artması üzerine kliniğimize başvurdu.Yapılan incelemeler sonucu, akciğerde pnömokonyoz ile uyumlu olabilecek yaygın parankimal infiltrasyonlar ve kronik tip 1 solunum yetmezliği saptandı. Özgeçmişinde bu duruma yol açabilecek mesleki maruziyet dışında, herhangi bir sigara içme veya hastalık öyküsü bulunmadığından olgunun diş teknisyeni pnömokonyozu olduğu kabul edildi. Diş teknisyeni pnömokonyozu patogenezinde, kullanılan karmaşık içeriğe sahip maddelere (metal tozları, silika, alçı, mum ve reçineler, likit uçucular, metil metakrilat) maruziyet ve bunların akciğer parankimine olan etkileri rol oynamaktadır. Metil metakrilat ile ilişkili EAA bildirilmiştir. Mesleksel akciğer hastalığı gelişimi için işyerindeki çoklu maruziyet en önemli faktördür. İşyerlerindeki yetersiz havalandırma koşulları ve korunma önlemlerindeki eksiklikler eklendiğinde risk daha da artmaktadır.
Dental technician’s pneumoconiosis; a case report
Since 1939, it has been known that, silicosis and extrinsic allergic alveolitis(EAA) can be seen among dental technicians. The interstitial disease caused by the exposure to complex substances used by dental technicians is classified as a special group called dental technician’s pneumoconiosis. A 36-year-old man, who has no smoking history, presented with severe dyspnea. He had worked in different dental laboratories for 22 years, but he did not have respiratory symptoms until 5 years ago. After that date, he had hospitalized and had been examined for respiratory pathologies for many times. He had came to our clinic, because of the progression of his dyspnea. Diffuse pulmonary parenchymal infiltrates which can be related with pneumoconiosis and chronical type 1 respiratory defficiency had been diagnosed as the result of the examinations. While he has no history of smoking or any other risk factors or diseases in his medical history, the case was accepted as dental technician’s pneumoconiosis. The factors related with the pathogenesis of dental technician’s pneumoconiosis are; the complex compound of the substances (metal dusts, silica, plaster, wax and recins, chemical liquids, methyl metacrilate) used in this sector and their effects on the lung parenchyma. EAA related with methyl methacrylate has been reported.The most important factor to acquire an occupational lung disease is a complex occupational exposure. The insufficient workplace airing and the lack of preventive measures added on this exposure, the risks become much more greater.
