Bron şiyal termoplasti; astımda yeni tedavi seçeneği

Bron şiyal termoplasti, maksimal medikal tedavi ile kontrol altına alınamayan orta ve a ğır astımlı hastaların tedavisinde hava yollarınaradyofrekans enerjisi kullan ılarak kontrollü şekilde termal enerji uygulayan ve hava yolu düz kas kütlesinin azaltılmas ın ı hedefleyennonfarmakolojik tedavi seçene ğidir. İşlem bilinçli sedasyon altında ayaktan, bir saatten daha kısa süren üçer hafta ara ile üç bronkos-kopi seansı şeklinde gerçekle ştirilir. Bron şiyal termoplasti ile, orta ve a ğır astımlı hastalarda ya şam kalitesinde düzelme, şiddetli alev-lenmelerde, acil servis ba şvurularında ve iş/okul günü kay ıplarında anlamlı azalmalar tespit edilmiştir. Hasta konforu ve güvenliği içindikkatli hasta yönetimi önemlidir. Bu derlemede, bron şiyal termoplasti hakkında yapılm ış klinik çalışmaların, etkinliğine dair kan ıtla-rın, mevcut protokollerin ve hasta yönetiminin güncel bilgiler ışığında gözden geçirilmesi amaçlanm ıştır.

Bronchial thermoplasty; a new treatment modality in asthma

Bronchial thermoplasty is a non-drug treatment modality for moderate-to-severe asthma that involves the delivery of radio frequencyenergy to the airway wall in a precisely controlled manner to reduce excessive airway smooth muscle. Bronchial thermoplasty isperformed under conscious sedation and completed in three bronchoscopy sessions, each lasting less than one hour, and eachspaced apart by about three weeks. Bronchial thermoplasty has been demonstrated to reduce severe exacerbations, emergencyrooms visits for respiratory symptoms, and time lost from work, school and other daily activities and improve asthma control andquality of life in patients with moderate-to-severe asthma.
