Alveoler mikrolitiyazis -kardeş 2 olgu nedeni ile

Pulmoner alveoler mikrolitiazis (PAM) alveollerde kalsiyum fosfat depolanması ile karakterizedir. PAM tanısı koyduğumuz iki kardeş (7 yaşında kız ve 13 yaşında erkek) olgudan kız kardeşte akciğer grafisi ve bilgisayarlı tomografide fibrotik, yumuşak nodüler değişiklikler saptandı. Kemik sintigrafisinde akciğer alanlarında tutulum yoktu. Bronkoalveoler lavajda Papanicolaou boyası ile pembe boyanan cisimler Von Kossa ile boyanmadı. Erkek kardeşin radyolojisinde tipik görünüm vardı. Kemik sintigrafisinde akciğer alanlarında tutulum saptandı. Bronkoalveoler lavajda Papanicolaou boyası ile pembe boyanan cisimler Von Kossa ile siyah boyandı. Birinci olgunun kemik sintigrafisinde akciğer alanlarında tutulum olmadığından erken dönemde PAM düşünüldü.

Alveolar microlithiasis -because of two siblings

Pulmonary alveolar microlithiasis (PAM) is characterized by deposition of calcium phosphate within the alveolar space. We reported two siblings (7 year old sister and 13 year old brother) with PAM. Chest X-ray and computed tomography of the sister revealed diffuse interstitial changes. No lung uptake with technetium 99m was detected on the bone scintigraphy. The particles stained pink with Papanicolaou dye in BAL fluid showed no staining with Von Kossa. In the brother, characteristic pattern was found on the chest X-ray and CT scan. Lung uptake was detected on bone scintigraphy. The particles that stained pink with Papnicolaou in BAL fluid were stained black with Von Kossa as well. In the first case we thought that it was an early stage disease because of the lack of lung uptake on the bone scan.
