Erzincan'da Kırk Yaş ve Üzeri Bireylerde Hipertansiyon Sıklığı, Farkındalığı, Tedavisi ve Kontrolü

AMAÇ: Bu çalışmanın amacı Erzincan il merkezinde bulunan 5 sağlık ocağına herhangi bir sebeple başvuran 40 yaş ve üzeri bireylerde hipertansiyon sıklığı, farkındalığı, tedavisi ve kontrolü durumunu belirlemektir. YÖNTEM: Kesitsel özellikte olan bu çalışma Ocak-Mayıs 2009 tarihleri arasında sağlık ocaklarına başvuran 40 yaş ve üzeri 1.570 kişi ile tamamlanmıştır. Veriler yüz yüze görüşerek toplanmış ve iki kez kan basıncı ölçümleri yapılmıştır. Sistolik kan basıncı ortalaması 140 mmHg ve üzeri olan veya diastolik kan basıncı 90 mmHg ve üzerinde olan veya antihipertansif ilaç kullanan bireyler hipertansif olarak kabul edilmiştir. Ayrıca bireylerin farkındalık, tedavi ve kontrol durumları değerlendirilmiştir. BULGULAR: Genel toplamda hipertansiyon sıklığı %67,0 (kadınlarda %69,6, erkeklerde %63,0) olarak belirlenmiştir. Çalışma örneğinde bireylerin %77,4’ünün kan basınçlarının yüksek olduğunun farkında olduğu, tamamının antihipertansif tedavi kullandığı ve bunların %33,9’unun kan basıncının kontrol altında olduğu belirlenmiştir. Tüm hipertansifler arasında kontrol %26,2 olarak saptanmıştır. Yaş, beden kitle indeksi (BKI), sigara kullanımı, eğitim düzeyi, tuz kullanımı ve diyabet ile hipertansiyon sıklığı arasında ilişki bulunmuştur. SONUÇ: Bu sonuçlara göre sağlık ocaklarına herhangi bir sebeple başvuran ve tedavi alan bireylerde hipertansiyon sıklığı yüksek ve yönetimi yetersizdir. Bireylerin önemli bir kısmı hasta olduğunun farkında değildir. Bu doğrultuda hipertansiyonun erken tanısı ve kontrolünün sağlanması, farkındalığın artırılmasına yönelik stratejilerin geliştirilmesine gereksinim vardır.

Frequency, Awareness, Treatment and Control of Hypertension in the over 40 Population of Erzincan

AIM: The aim of this study is to determine the frequency, awareness, treatment and control of hypertension in population over 40 years of age who came to the 5 primary health care centers in Erzinca for any reason. METHODS: This cross-sectional study was completed with 1570 people aged 40 and older, who received care at health care centers between January and May 2009. The data were collected through face-to-face interviews, and blood pressures were measured twice. Participants were considered as hypertensive if thei average systolic blood pressure was ≥140 mmHg, or their average diastolic blood pressure was ≥90 mmHg, or if they were taking antihypertensive medication. The awareness, treatment and control status o the individuals were also assessed. RESULTS: The overall hypertension frequency was 67.0% (69.6% in women and 63% in men). In the study sample, 77.4% of individuals were aware of their hypertension, all were receiving anti-hypertensive drugs, and 33.9% of patients had their blood pressure under control. Among all hypertensive individuals, the percentage of controlled hypertension (HT) was 26.2%. The frequency of hypertension was associate with age, body mass index (BMI), smoking, level of education, the use of salt, and diabetes. CONCLUSION: These results indicate that the frequency of HT was high, and it was inadequately managed in individuals who came to the primary healthcare centers for any reason and received treatment Many of the individuals were not aware of their illness. Therefore, it becomes very important and necessary to ensure early diagnosis and control of hypertension and to develop strategies to increase awareness.
