Koklear implantlı çocuklarda uzun dönem vestibüler uyarılmış miyojenik potansiyel yanıtları

Amaç: Bu çalışmada koklear implant cerrahisi uygulanan hastalarda vestibüler sistemin işlevleri vestibüler uyarılmış miyojenik potansiyeller VEMP üzerinden uzun dönemde değerlendirildi.Hastalar ve Yöntemler: Çalışmamıza koklear implant cerrahisi sonrası en az bir yıldır takip edilen 18 yaş altı 35 hasta 19 erkek, 16 kız; ort. yaş 13.0±3.6 yıl; dağılım 5-18 yıl dahil edildi. Kontrol grubu, öyküsünde vestibüler sistemle ilgili herhangi bir yakınması olmayan 35 gönüllü 18 yaş altı sağlıklı birey 21 erkek, 14 kız; ort. yaş 14.6±3.2 yıl; dağılım 5-18 yıl içerdi. Çalışma ve kontrol grubunun her iki kulaklarından elde edilen servikal ve oküler VEMP yanıtları gruplar arasında karşılaştırıldı.Bulgular: Koklear implantlı çocuklarda servikal VEMP dalgası elde edilme oranları ameliyat edilen ve karşı taraf kulaklar arasında istatistiksel olarak anlamlı bir farklılık göstermedi p=0.802 . Koklear implantlı çocuklarda ameliyat edilen ve karşı taraf kulaklar arasında oküler VEMP dalgası elde edilme oranlarında da anlamlı bir farklılık yoktu p=0.078 . Kontrol grubundaki çocukların kulaklarında servikal VEMP ve oküler VEMP dalgaları, implantlı hastaların hem ameliyat edilen hem karşı taraf kulaklarına kıyasla anlamlı olarak daha yüksek oranda elde edildi p

Long-term vestibular evoked myogenic potential responses in children with cochlear implant

Objectives: This study aims to evaluate the long-term functions of vestibular system via vestibular evoked myogenic potentials VEMPs in patients who were administered cochlear implant surgery. Patients and Methods: Our study included 35 patients aged under 18 years 19 males, 16 females; mean age 13.0±3.6 years; range 5 to 18 years who were followed-up for at least one year after cochlear implant surgery. Control group included 35 voluntary individuals aged under 18 years 21 males, 14 females; mean age 14.6±3.2 years; range 5 to 18 years , who had no history of any complaint related to the vestibular system. Cervical and ocular VEMP responses acquired in both ears of study and control groups were compared between the groups. Results: Rates of cervical VEMP wave acquisition in children with cochlear implants did not show any significant difference between the operated and opposite side ears p=0.802 . In children with cochlear implants, there was also no significant difference between the operated and opposite side ears in terms of rates of ocular VEMP wave acquisition p=0.078 . In the ears of children in the control group, cervical VEMP and ocular VEMP waves were acquired at significantly higher rates compared to those in both operated and opposite side ears of children with implants p<0.001 . Conclusion: In this study, VEMP testing impairments were detected in long-term follow-ups of patients with cochlear implants compared to healthy controls. Some of the VEMP testing impairments may be related to the primary diagnosis leading to the implant requirement, while others may be secondary to surgery-related vestibular damage. Preoperative vestibular tests should also be performed particularly in patients for whom bilateral cochlear implantation is planned.
