Postmenopoz Dönemdeki Kadınlarda Sağlık İnanç Modeli Temelli Kısa Görüşmelerin Osteoporozdan Korunma Bilgi, Tutum ve Davranışlarına Etkisi
Osteoporoz, günümüzde gittikçe artan ve toplumun büyük bir kesimini etkileyen önemli bir halk sağlığı problemidir. Uluslararası Osteoporoz Vakfına (International Osteoporozis Foundation) göre, dünyada 34 milyon kişinin osteoporoz hastalığı ile karşı karşıya olduğu tahmin edilmektedir. Osteoporozun en önemli komplikasyonu kırıklara neden olmasıdır. Özellikle kalça kırığı yaşayanlarda mortalite oranı ilk iki yıl içinde %12-20’dir. Dünya’da osteoporozdan etkilenen kişilerin yaklaşık olarak %40’ını postmenopoz dönemdeki kadınlar oluşturmaktadır. Dünya Sağlık Ö̈rgütü tarafından osteoporoz mortalite ve morbidite açısından, postmenopoz dönemdeki kadınlar için kardiyo- vasküler risklerden sonra ikinci sırada gösterilmektedir. Osteoporozdan korunma, birinci basamak sağlık hizmetleri içinde önemli bir yer tutmaktadır. Hemşirelerin postmenopoz dönemdeki kadınların sağlık inançlarında olumlu davranış değişikliği oluşturmak amacıyla planlanmış sağlık eğitim çalışmaları bulunmaktadır. Bu çalışmalarda kadınların diyette kalsiyum alma oranında, egzersiz yapma davranışlarında olumlu gelişmeler olduğu belirtilmektedir. Birinci basamak sağlık hizmetlerinin yürütülmesinde hemşirelerin hizmet verdiği gruplara kaliteli zaman ayırma konusunda sınırlılıklar yaşayabileceği düşünülmektedir. Hemşireler aile hekimliğini sık kullanma olasılığı bulunan postmenopoz dönemdeki kadınları, osteoporozdan koruyabilmek için onların sağlık inanç ve tutumlarını belirleyebilir özellikle beslenme, egzersiz yapma güneşlenme gibi kemik sağlığını korumaya yönelik bilgi ve davranışları konusunda motive edebilir.
The Role of Nurse in Prevention of Postmenopausal Osteoporosis and in Changing Patients’ Health Beliefs
Osteoporosis is an important and ever-increasing public health problem which affects a large part of the society today. According to the International Osteoporosis Foundation, it is estimated that 34 million people worldwide are suffering from osteoporosis. The most important complication of osteoporosis is bone fractures. The mortality rate among people, particularly among those who experienced hip fracture is 12-20% in the first two years. Approximately 40% of people affected by osteoporosis in the world are postmenopausal women. According to the World Health Organisation, osteoporosis is the second most common cause of mortality and morbidity among postmenopausal women after cardiovascular risks. Osteoporosis prevention has an important place in primary healthcare. There are health education studies for nurses planned in order to promote positive behaviour changes in the health beliefs of postmenopausal women. In these studies, it is remarked that there are positive changes in dietary calcium intake and exercise behaviours of women. It is thought that nurses may experience some constraints in devoting quality time to patient groups during primary healthcare services. Nurses may shape the health beliefs and attitudes of the postmenopausal women who are likely to utilize family medicine frequently, in order to protect them from osteoporosis. In particular, nurses can motivate them regarding their knowledge and behaviours such as nutrition, exercise, sunbathing to protect bone health.
