Copeman Nodülüne Bağlı İnatçı Bel Ağrısı: Vaka Bazlı Bir İnceleme

Copeman nodülleri (CN), klinik uygulamalarda çoğunlukla ihmal edilen, bel ağrısının potansiyel olarak tedavi edilebilir bir nedenini temsil etmektedir. Çeşitli merkezlere başvuran ama tanı konulamayan kronik bel ağrısı şikayeti olan bir hasta üzerinden konu tartışılmıştır. Hastada fizik muayene ve Ultrasonografi görüntüleme sonucunda CN olduğu tespit edilmiştir. Hastada nodül içine lokal anestezik + steroid kombinasyonu enjekte edilmiştir. Enjeksiyondan hemen sonra hastanın bacağına yayılan ağrısı tamamen kaybolurken bel ağrısı Visuel Analog Skala’ya (VAS) göre VAS 8’den VAS 2’ye geriledi. 2 hafta sonra hastanın ağrısı VAS 1’e geriledi. Bu olgudan yola çıkarak kronik bel ağrısı olan hastalarda CN akla gelmelidir ve palpasyon ve fizik muayenenin tanı açısından çok önemli olduğu unutulmamalıdır ayrıca hastalar bu açıdan değerlendirilmezse çok kolay tedavi edilebilen bir durum nedeniyle uzun süre ağrı çekebilirler ve gereksiz testlere tabi tutulabilirler.

Persistent Back Pain Due to Copeman's Nodule: a Case-Based Review

Copeman's nodules (CN) represent a potentially treatable cause of low back pain that is often neglected in clinical practice. The subject was discussed through a patient with chronic low back pain who applied to various centers but could not be diagnosed. The patient was found to have CN as a result of physical examination and ultrasonography imaging. Local anesthetic + steroid combination was injected into the nodule in the patient. Immediately after the injection, the patient's pain radiating to his leg completely disappeared, while his low back pain decreased from VAS 8 to VAS 2 according to the Visual Analogue Scale (VAS). After 2 weeks, the patient's pain regressed to VAS 1. Based on this case, CN should be considered in patients with chronic low back pain, and it should not be forgotten that palpation and physical examination are very important in terms of diagnosis, and if patients are not evaluated in this respect, they may suffer from pain for a long time due to a very easily treatable condition and may be subjected to unnecessary tests.
