Üst dudak ve damakta lokalize pleomorfik adenom: İki olgu sunumu
Pleomorfik adenom tükürük bezlerinin en yaygın iyi huylu tümörüdür. Tümör damak, üst dudak, yanak, ağız tabanı, larinks ve trakeada görülebilir. Lezyonun tedavisi cerrahi olarak lezyonun çıkarılmasıdır. Cerrahi esnasında rezeksiyon geniş yapılmazsa ya da lezyonun kapsülü rüptüre olursa nüks edebilir. Bu çalışmanın amacı iki farklı klinikte tedavi edilmiş iki pleomorfik adenom vakasını ve tedavi seçeneklerini sunmaktır. Olgu 1: 31 yaşında erkek hasta Mustafa Kemal Üniversitesi Diş Hekimliği Fakültesi Ağız-Diş ve Çene Cerrahisi kliniğine üst dudakta şişlik şikâyetiyle başvurdu. Hastanın klinik muayenesinde üst dudağının iç kısmında lokalize mobil, ağrısız kitle görüldü. Kitlenin yaklaşık iki yıldır mevcut olduğu öğrenildi. Lokal anestezi altında mukoza insizyonu yapılıp kitlenin tamamı çıkarıldı. Yapılan histopatolojik incelemeden sonra kitlenin pleomorfik adenom olduğu anlaşıldı. Hastanın 1 yıllık takibinde herhangi bir problemle karşılaşılmadı. Olgu 2: 23 yaşında erkek hasta Gaziantep Üniversitesi Diş Hekimliği Fakültesi Ağız-Diş ve Çene Cerrahisi kliniğine damakta var olan şişlik şikâyetiyle başvurdu. Hastanın yapılan muayenesinde sağ palatinal molar bölgede şişlik görüldü. İnce iğne aspirasyon biyopsisi sonucunda lezyonun pleomorfik adenom olduğu öğrenildi. Lezyon lokal anestezi altında çıkartıldı. Yara bölgesi sekonder iyileşmeye bırakıldı. Çıkarılan lezyonun histopatolojik sonucu ilk teşhisi doğruladı. Hastanın üç aylık takibinde herhangi bir komplikasyon görülmedi. Pleomorfik adenom iyi huylu bir tümör olmasına rağmen nüks ya da malign transformasyon gösterebilir. Bu nedenle lezyon teşhis edildikten sonra cerrahi olarak çıkartılmalı ve hastalar uzun süre takip edilmelidir.
Pleomorphic adenoma located in the upper lip and hard palate: Two cases
The pleomorphic adenoma is the most common benign tumor of the salivary glands. The tumor occurs in the palate, upper lip, cheek, floor of the mouth, larynx and trachea. The surgical treatment for the pleomorphic adenoma includes complete wide surgical excision. Inadequate resection or rupture of the capsule can lead to local recurrence. We present two different pleomorphic adenoma cases located in the upper lip and hard palate and their treatment choices.Case Report 1: A-31-year old man was referred to the Department of Oral and Maxillofacial Surgery, Faculty of Dentistry, Mustafa Kemal University with a complaint of a slowly growing painless swelling in his left half of the upper lip of two years duration. A mobile painless mass was located under the mucosa. A mucosa incision was made under local anesthesia. At surgery, the solid mass was encapsulated, and was completely excised. The patient followed up for 12 months and no recurrences were observed.Case Report 2: A-23-year old man was referred to the Department of Oral and Maxillofacial Surgery, Faculty of Dentistry, Gaziantep University with a complaint of a slowly growing painless swelling in the hard palate. After fine-needle aspiration biopsy, we learned that the lesion was pleomorphic adenoma. Under local anesthesia we made resection widely. We follow up the patient for 3 months and no recurrences were observed. The pleomorphic adenoma is a benign tumor, but it can metastasize when enucleation or incomplete excision is performed.
