Yoğun Bakım Ünitesinde Yatan Hastaların Çeşitli Klinik Örneklerinden izole edilen Vankomisine dirençli enterokok (VRE) suşlarının teikoplanin, linezolid, daptomisin ve tigesiklin duyarlılıklarının E-test yöntemiyle belirlenmesi
Amaç: Enterokoklar son yıllarda yoğun bakım ünitelerinde yatan hastalarda rektal kolonizasyona ve enfeksiyonaneden olabilen önemli bir nozokomiyal enfeksiyon etkenidir. Bazı yayınlarda nozokomiyal bakteriyemilerde ikincisıklıkta izole edildiğini bildirilmiştir. Enterokokların neden olduğu enfeksiyonların tedavisinde, penisilin direnci yok- sa beta-laktam ve aminoglikozid grubu kombinasyonu, eğer penisilin direnci varsa glikopeptid grubu antibiyotiklertercih edilir.Ancak, hastane kaynaklı enterokok izolatlarının çoklu antibiyotik direnci ve yüksek düzey aminoglikozid direnci(YDAD) göstermesi yanı sıra vankomisin direncin de artış olması tedavide önemli bir sorundur. Vankomisinedirençli enterokokların tedavisinde kullanılabilecek başlıca antibiyotikler; linezolid, daptomisin, tigesiklin vekinopristin-dalfopristindir. Bu çalışmanın amacı, Etlik İhtisas Eğitim ve Araştırma Hastanesinde Ocak 2010-Mart 2012 yıllarında Yoğun BakımÜnitesinde yatan 27 hastanın çeşitli klinik örneklerden (25 rektal sürüntü, 2 kan, 1 idrar örneği) izole edilen toplam28 vankomisine dirençli enterokok (VRE) suşunun teikoplanin, linezolid, daptomisin ve tigesiklin için minimalinhibitör konsantrasyon (MİK) değerlerinin E-test yöntemiyle belirlenmesidir. Yöntem ve Gereçler: Çalışmaya yoğun bakım ünitesinde yatan 27 hastanın rektal sürüntü (n: 25), kan kültürü (n:2) ve idrar kültürü (n: 1) örneklerinden izole edilen toplam 28 VRE suşu dahil edildi. Susların teikoplanin, linezolid,daptomisİn ve tigesiklin için MİK değerleri ticari E-test stripleri (Biomerieux, France) stripleri ile üretici firmanınönerileri doğrultusunda çalışıldı.Bulgular: Teikoplanin, linezolid, daptomisin ve tigesiklin için MİK değeri konsantrasyon aralığı 0.016-256 µg /ml idi. Vankomisine dirençli enterokok suşlarında test edilen antibiyotiklerden teikoplanine %100 oranında dirençsaptanırken, linezolid, daptomisin ve tigesikline direnç saptanmadı.Suşların MİK50 ve MİK90 değerleri sırasıyla; teikoplanin için 32 µg /ml ve 256 µg /ml, linezolid için 0,5 µg /ml ve 1 µg /ml, daptomsin için 0,38 µg /ml ve 1 µg /ml, tigesiklin için 0,064 µg /ml ve 0,25 µg /ml olarak belirlendi. Çalışılan antibiyotikler içerisinde VRE suşlarına en etkili antibiyotikler sırasıyla; tigesiklin, daptomisin velinezolid olarak belirlendi.Sonuç: Çalışmamızda yoğun bakım hastalarından izole edilen VRE suşlarında teikoplanine direnç saptanmasınedeniyle, VRE enfeksiyonlarının tedavisinde tigesiklin, daptomisin ve linezolid gibi in vitro duyarlılık oranlarıyüksek antibiyotiklerin kullanılması, ampirik tedaviye karar verirken ise hastanenin antibiyotik direnç sürveyansverilerinin göz önüne alınması uygun olacaktır.
The Determination of Antibiotic Susceptibilities of Vancomycine Resistant Enterococci (Vre) Strains Isolated From Various Clinical Materials of the Patients in the Intensive Care Unit toTeicoplanin, Linelozid, Daptomycin and Tigecycline By The E-Test Method
Objectives: Aim: Enterococci have recently been claimed to be a nasocomial infection agent that may cause rectalcolonization and infection in intensive care unit patients. In some reports, it is the second most frequently isola- ted pathogen in nasocomial infections. In the treatment of enterococcal infections, beta lactam and aminoglycosidecombination is the treatment of choice in the absence of penicillin resistance. Otherwise, glycopeptides are pre- ferred. However, hospital acquired enterococci not only have high level of aminoglycoside or multiple antibioticresistance, but also have an increased rates of vancomycin resistance in recent years. The other choices in case ofvancomycine resistance are linezolide, daptomycine, tigecycline and quinupristine-dalfopristin. We aimed to determine the minimum inhibitory concentrations (MIC) of vancomycine resistant enterococci(VRE) strains for teichoplanin, linelozid, daptomycin and tigecycline that were isolated from various clinicalsamples (rectal swab, blood, urinary) of patients in the intensive care unit. Materials and Methods: We included a total of 28 VRE strains that were isolated from rectal swab (n= 25), bloodculture (n= 2) and urinary culture (n=1) samples of 27 patients between January 2010 and March 2012. Minimuminhibitory concentrations of VRE strains for teichoplanin, linelozid, daptomycin and tigecycline were evaluated bycommercial E-test strips (Biomerieux, France) according to the recommendations of the manufacturer company.The MIC range for teicoplanin, linelozid, daptomycin and tigecycline was between 0.016-256 µg /ml.Results: The resistance rate was 100% for teicoplanin whereas no resistance was observed to linelozid, daptomycinand tigecycline. The MIC50 and MIC90 values of strains were 32 µg/ml and 256 µg/ml for teichoplanin, 0,5 µg/ ml and 1 µg/ml for linezolid, 0,38 µg /ml and 1 µg /ml for daptomycine, and 0,064 µg/ml ve 0,25 µg/ml for tigecy- cline; respectively. The most effective antibiotics for VRE strains were tigecycline, daptomycine and linezolid, indescending order. Conclusion: In this study, we observed resistance to teichoplanin in VRE strains. Accordingly, in our opinion,prescription of antibiotics with high in vitro sensitivities as tigecycline, daptomycine and linezolid may be the besttherapeutic option for the treatment of VRE infections. When deciding on empiric treatment, hospital s antibioticresistance surveillance data would be take into account.
