Transrektal Prostat İğne Biyopsisi Sonrasında Görülen Üriner Sistem Enfeksiyonlarında ve Direnç Gelişiminde Biyopsiden Önce Kinolon Kullanılması Risk Faktörü müdür?
Amaç: Bu çalışmada biopsi sonrası üriner sistem enfeksiyonu gelişen hastalarda en az 2 hafta önceki kinolonkullanımının, infeksiyonda direnç gelişmesi açısından etkisi olup olmadığını değerlendirmeyi amaçladık. Yöntem ve Gereçler: Çalışmada Ocak 2009- Ekim 2011 tarihleri arasında transrektal USG eşliğinde prostat biy- opsisi yapılan 68 hastanın verileri retrospektif olarak değerlendirildi. Biyopsiden iki hafta önce kinolon tedavisialmayan hastalar (grup 1:53) , kinolon tedavisi alan hastalar (grup 2: 15) olarak ayrıldı. Tüm hastalara işlem günüproflaksi amacıyla ve takibinde 500 mg 2 tablet siprofloksasin ve 500 mg metranidazol beş gün önerildi. İşlem so- nucunda dizüri, ateş, genel durum bozukluğu, hematüri şikayetleri olan olgular hemen, diğerleri ise 7.gün ürolojipolikliniğine kontrole çağırıldı. Bulgular: Hastalarımızın yaş ortalaması 66,4 yıl, ortalama PSA değerleri 1. Grup için 12,3, 2. Grup için 12,9 idi(p=0,9). Çalışmaya katılan hastalardan 15 (%22)i biyopsi öncesi üriner sistem enfeksiyonu veya prostatit ön tanısıyla2 hafta önceye kadar antibiyotik tedavisi kullanmıştı. Çalışmaya katılan hastalardan üçünde semptomatik üriner en- feksiyon gelişti. Grup 1de iki (%4) hastanın, grup 2de ise bir (%7) hastanın idrar kültüründe ESBL pozitif E.coliüredi. Gruplar arasında enfeksiyon açısından istatistiksel fark saptanmadı.Sonuç: İki hafta önce kullanılan kinolon tedavisinin üriner sistem infeksiyonunda kinolon direnci açısından biy- opsi sonrasında riski arttırmadığını saptadık. Fakat bu konu ile ilgili daha geniş hasta serileri ile yapılan prospektifçalışmalara ihtiyaç olduğu da göz önünde bulundurulmalıdır.
Urinary Tract Infections And Antibotic Resistance Appear After Prostate Needle Biopsies: Is Flo- uroquinolon Use Prior to Biopsies A Risk Factor?
Aim: Transrectal prostate biopsy (TRPB) can cause urinary tract infections (UTI). We aimed to study whether havingused kinolons for UTI s 15 days before biopsies had risk on infections after TRPB.Material And Method: 68 patients had TRPB were retrospectively evaluated. Patients were divided into two groupsas not having used flouroquinolons prior to TRPB (group 1: n: 53) and as having used kinolons for UTI prior to TRPB(group 2: n: 15). All patients had ciprofloxasin 500 mg twice a day and metronidazole 500 mg once a day for 5 days.Having symptoms such as disuria, fever, hematuria were evaluated immediately, the others called at day 7. Results:The mean age of patients was 66,4 years, mean PSA was 12,3 and 12,9 for groups 1 and 2 respectively. 15(22%) of patients received antibiotherapy for UTI or prostatitis. 3 of all patients suffered symptomatic UTI. 2 patients(4%) in group 1 and 1 patient in group 2 (%7) had ESBL positive E.coli in urine cultures. There was no statisticaldifference between groups. Conclusion: Recent use of flouroquinolons prior to prostate biopsy had no risk of resistance. However prospectivestudies conducting on larger populations are needed to confirm the results.
