0-5 Yaş arası akut gastroenteritli çocuklarda rotavirüs ve adenovirüs sıklığının belirlenmesi
Amaç: Viral gastroenteritler infeksiyöz morbiditede solunum yolu infeksiyonlarından sonra oldukça önemli rol oyna- maktadır. Bu çalışmada hastanemize başvuran 0-5 yaş arası akut gastroenteritli çocuk hastalarda rotavirüs ve adeno- virüs enfeksiyon sıklığının ve bu enfeksiyonların yaş ve mevsimsel özelliklere göre değişiminin belirlenmesi amaç- lanmıştır. Yöntem ve Gereçler:1 Ocak 2011- 31 Aralık 2011 tarihleri arasında çocuk polikliniği ve çocuk acil polikliniğine baş- vuran, gaita mikroskopisi ile birlikte gaitada rota-adeno virus araştırılması istenen 0-5 yaş arası 5215 hasta değerlen- dirmeye alındı. Gaitada rota-adeno virus antijenlerinin varlığı kalitatif immünokromatografik yöntem (RIDA Quick Rota-Adeno Kombi R-Biopharm AG, Germany) ile üretici firmanın çalışma prosedürüne uygun olarak araştırılmıştır. Bulgular:Toplam 5215 hastanın gaita örneği incelenmiştir. Rotavirus antijen pozitifliği %17.8 adenovirus antijen pozitifliği %3.9 olarak saptanmıştır. Erkek çocuklarda, gaitada hem rotavirus antijen pozitiflik oranı (%54.2) hem de adenovirus antijen pozitiflik oranı (%57.5) daha sık olarak gözlenmiştir. Gaitada hem rotavirüs antijeni pozitifli- ği (%44.80) hem de adenovirus antijen pozitifliği (%53.6) en sık 0-1 yaş aralığında bulunmuştur. Ayrıca rotavirus ve adenovirus olguları en sık kış ve sonbahar aylarında görülmüştür. Adenovirus olguları ise rotavirus olgularına göre yazın daha sık saptanmıştır. Sonuç:Viral gastroenteritlerin erken tanısı ve sürveyans çalışmalarıyla takip edilmesi bu infeksiyonlara bağlı mortalite ve morbiditenin azaltılmasında önemli rol oynar. Ayrıca tanının konması, bakterilerde son yıllarda giderek artan antibiyotik direncinin başlıca nedeni olan gereksiz antibiyotik kullanımının önlenmesi açısından da çok önemlidir. Gaita örneklerinde rotavirus ve adenovirus özellikle sık görüldüğü aylarda gastroenterit tanısıyla başvuran 0-5 yaş arası çocuklarda rutin olarak araştırılmalıdır.
Determination of rotavirus and adenovirus frequency ın children with gastroenteritis between 0-5 years of age
Aim:Viral gastroenteritis are playing very important role in infectious morbidity after the respiratory infections. In this study we aimed to determine the frequency of rotavirus and adenovirus infections at 0-5 ages children who ap- plied to our hospital due to acute gastroenteritis, and changes according to age and seasonal features of these infec- tions. Material And Methods: 5215 patients between 0-5 ages who applied to pediatry clinics and emergency pediatry clinics for gaita microscopy together with rota-adeno virus inspections between January 1, 2011- December 31, 2011, have been evaluated. Existence of the rota-adeno virus antigens at stool specimens has been investigated by using qualitative immunochromatographic method (RIDA Quick Rota-Adeno- Kombi R- Biopharm AG, Germany) in ac- cordance with the study procedure of the producer company. Results:Totally 5215 patients’ stool specimens were investigated. Rotavirus antigen positivity was 17.8%, adenovirus antigen positivity was 3.9%, rota-adenovirus antigen positivity was 1.07%. It has been observed that both rotavirus antigen ratio (54.2%) and adenovirus antigen ratio (57.5%) at stool specimens are more common at boys. It has been found out that both rotavirus antigen positivity ratio (44.89%) and adenovirus antigen positivity ratio (53.6%) at stool specimens are more common at 0-1 age group. Besides, rotavirus and adenovirus cases have been most commonly seen during winter and fall. Adenovirus cases have been detected more commonly during summer time compared to rotavirus cases. Conclusion:Early diagnosis and observation by surveillance studies of the viral gastroenteritis plays important role in reducing the morbidity and the mortality cases affiliated with these infections. In addition, diagnosis is also important in preventing unnecessary usage of antibiotics, which is the main reason of increasing antibiotic resistance in bacteria in recent years. Rotavirus and adenovirus at stool specimens should be routinely investigated especially at the 0-5 ages children who applied for gastroenteritis during the frequently observed seasons.
