In this case, we describe a 43 year old female who presented with serious fatigue, progressive muscle weakness and difficulty in walking. Initially, we consider that neurological disorder may be due to her clinical signs and symptoms. Therefore, our investigations were conducted primarily to rule out neurological diseases. But, imaging, electromyography findings and laboratory measurements showed that we faced with Hoffmann’s Syndrome relationship hypothyroid myopathy, which rare cause muscular pseudohypertrophy. Clinical and laboratory improvement was observed with thyroxine treatment
