Rabdomiyolize bağlı gelişen akut tubuler nekrozda kas kitlesinin önemi: Olgu sunumu
Acil servise sol üst kadran ağrısıyla başvuran 30 yaşında erkek hastanın öyküsünden öğrenildiği kadarıyla yaklaşık bir gün önce bir sokak kavgasına karıştığı, kavga sırasında demir sopalarla ve tekme ile göğsüne, sırtına ve sağ koluna darp edildiği öğrenildi. Yapılan fizik incelemede göğüs, sırt ve sağ kolda yer yer küçük ekimotik alanlar ile sol üst kadranda palpasyonla hassasiyet saptandı. Laboratuar incelemelerinde kreatin fosfokinaz, kreatinin ve potasyum düzeyleri yüksek bulundu. Hasta, rabdomiyolize bağlı akut tubuler nekroz tanısıyla acil servise yatırıldı. İzotonik NaCl, sodyum bikarbonat ve furosemid tedavisi ile yatışının dördüncü gününde serum değerlerinin normal sınırlara gerilediği gözlendi. Travmada etkilenen kas kitlesinden bağımsız olarak serumda kas yıkım ürünlerinin oldukça yüksek bulunması akut tubuler nekroz olasılığını da beraberinde getirmektedir. Travma nedeniyle acil servise başvuran hastalarda etkilenen kas kitlesine bakılmaksızın serum CPK, Cr ve K+ değerleri incelenmeli, yüksek bulunan olgularda oluşabilecek komplikasyonların önlenebilmesi için tedaviye erken başlanmalıdır.
Importance of muscle mass in acute tubular necrosis due to rhabdomyolisis: Case report
30 years old male patient presented to our emergency clinic with complaint of left upper quadrant pain. We learned there has been a fight, he was assaulted with fists and iron bricks to his abdomen and right arm. Clinical examination revealed echymotic areas in his thorax, back and right arm; tenderness at left upper quadrant of abdomen. In laboratuary tests creatinin phosphokinase (CPK), creatinin(Cr) and potassium(K) levels were high. The patient admitted to emergency service with diagnosis of rhabdomyolysis. We started izotonic saline infusion, sodium bicarbonate and furosemide treatment. His serum levels returned to normal values in the fourth day of his admission. In a trauma patient independent from extent of muscle damage if enzymes are high; a probability of acute tubuler necrosis risk is also high. In patients presented to emergency clinic due to trauma, without looking at the extent of muscle injury ; CPK, Cr, K levels must be seen, and if they are high , to prevent complications treatment must be started immediately.
