Preoperatif Vajinal Lavaj ve Tek Doz Antibiyotik Profilaksisinin Postoperatif Enfeksiyonlar Üzerine Etkisi
GİRİŞ ve AMAÇ: Çalışmamızda preoperatif vajinal lavaj ve tek doz antibiyotik profilaksisinin histerektomi sonrasında postoperatif enfeksiyöz morbiditeye olan etkileri araştırıldı. Ayrıca kısa dönem uygulanan antibiyotik profilaksisi ve vajinal povidone-iodine lavajının postoperatif vajinal floraya etkilerinin olup olmadığı değerlendirildi. YÖNTEM ve GEREÇLER: Çalışmamız benign nedenlerle total abdominal histerektomi ve bilateral salpingo - ooferektomi yapılan 36 hastada gerçekleştirildi. Hastalar 4 gruba ayrıldı: grup A; seftizoksim profilaksisi alan ve povidone-iodine ile vajinal yıkama yapılan, grup B; seftizoksim profılaksisi alan ve serum fizyolojik ile vajinal yıkama yapılan, grup C (Kontrol); seftizoksim almayan ve vajinal yıkama yapılmayan, ve grup D; seftizoksim profilaksisi alan ve vajinal yıkama yapılmayan grup. Gruplar postoperatif enfeksiyon gelişimi, vajinal flora, ateş ve hospitalizasyon süreleri açısından karşılaştırıldı. BULGULAR: Seftizoksim profilaksisi alan A, B, D grubu hastalarda yara yeri enfeksiyon oranı %0, kaf sellüliti oranı %3.7 olarak tespit edildi. Antibiyotik profilaksisi almayan C grubu hastalarda yara yeri enfeksiyonu oranı %22.2, kaf sellüliti oranı %11.1 olarak bulundu. The efficacy of preoperative vaginal wash and single-dose antibiotic prophylaxis on the postoperative infectious morbidity
INTRODUCTION: In our study we evaluated the efficacy of preoperative vaginal wash and single dose antibiotic prophylaxis on the postoperative infectious morbidity after hysterectomy. Also it was appraised that the effectiveness of antibiotic prophylaxis and vaginal povidone-iodine wash and the postoperative vaginal flora. METHODS: We studied on 36 women who were undergoing total abdominal hysterectomy and bilateral salpingooopherectomy for benign reasons. The patients were divided into 4 groups: group A; seftizoksim prophylaxis and vaginal wash with povidone-iodine, group B; seftizoksim prophylaxis and vaginal wash with serum physiologic, group C (Control); without seftizoksim prophylaxis and vaginal wash, and group D; seftizoksim prophylaxis but no vaginal wash. Postoperative infections, vaginal flora, fever and hospitalization times were compared between the groups. RESULTS: The operative-site infection occured in %0 and the cuff cellulitis occured in %3.7 of the women in groups A, B and D who were given seftizoksim prophylaxis. The operativesite infection occured in %22.2 and the cuff cellulitis occured in %11.1 of the women in group C who were not given antibiotic prophylaxis.
