Mastalji Şikayeti ile Başvuran Hastalarda Etiyolojik Faktörler
GİRİŞ ve AMAÇ: Mastalji, kadınların yaklaşık 2/3’ününhayatlarının bir dönemine doktora başvurmasına sebep olanşikayet sebebidir. Meme başı ağrısının yanı sıra memelerdenbiri veya ikisindeki ağrı duyusu olarak tanımlanır. Mastaljietiyolojisi ile ilgili literatürde birçok predispozan faktörliteratürde sorumlu tutulmuştur. Özellikle hormonal faktörlerinbaşında östrojen, progesteron ve prolaktin seviyelerindekideğişimler tartışılmıştır. YÖNTEM ve GEREÇLER: Ocak 2016 – Aralık 2017 tarihleriarasında kadın doğum polikliniklerinden yönlendirilen vegenel cerrahi polikliniğine memede ağrı şikayeti ile başvuranve çekilen mamografi ve ultrasonografilerinde herhangi birpatoloji saptanmayan hastaların dosya ve arşiv kayıtlarıtarandı. BULGULAR: Çalışmamıza dahil edilen 115 hastanın yaşortanca değeri 37 (18-70) idi. Yedi hastada ailede memekanseri öyküsü anamnezi vardı.Yüz bir hasta ev hanımı iken,68 hasta ilkokul mezunu idi. Hastaların %45.2’sine ağrılar 24saatin üzerinde idi. Günlük ağrı süreleri incelendiğinde en sıknadiren ağrı ve günlük 1-2 saat süren ağrı şikayetlerininolduğu gözlendi.Yirmi dokuz hastanın mastaljisinin menstrüelsiklus ilişkili olduğu tespit edildi.On yedi hasta menapozda idi.Sadece3 hastanın özgeçmişinde meme cerrahi geçirmiş olduğuöğrenildi TARTIŞMA ve SONUÇ: Her ne kadar daha yüksekpopülasyonlu hasta gruplarına ihtiyaç duysak ta çalışmamızındüzenlendiği bölge itibarı ile sosyo-kültürel düzeyi düşük olanlokasyonda, geçmişinde psikiyatrik tedavi görmüş olanhastaların sekonder kazanç sağlama amaçlı mastaljişikayetleri le hekime başvurdukları sonuca ulaştık
Etiological Factors in Patients Presenting With Mastalgia
INTRODUCTION: Mastalgia is the reason for complaintsthat cause about 2/3 of women to see a doctor for some periodof their lives. It is defined as the sense of pain in one or both ofthe breasts, as well as nipple. Many predisposing factors havebeen implicated in the literature on the etiology of mastalgia.Forthe hormonal factors, especially changes in estrogen,progesterone and prolactin levels are reported. METHODS: The files and archive records of patients whowere referred from the obstetric outpatient clinics betweenJanuary 2016 and December 2017 and who applied to thegeneral surgery outpatient clinic with complaints of breastpain and who did not find any pathology in theirmammography and ultrasonography were scanned RESULTS: The median age of 115 patients who were includedin our study was 37 (18-70). Seven patients had a familyhistory of breast cancer. One hundred patients were housewives, while 68 patients were primary school graduates.Pain was over 24 hours in 45.2% of the patients. When dailypain periods were analyzed, it was observed that the mostfrequent pain was rarely and 1-2 hours of pain complaints perday were observed. Twenty nine patients' mastalgia was foundto be related to the menstrual cycle. DISCUSSION AND CONCLUSION: Although we needhigher populations of patients, we have reached the conclusionthat patients who have undergone psychiatric treatment in thepast have applied to the physician with mastalgia complaintsfor secondary gain in the location where the socio-culturallevel was low.
