Kedi ve köpeklerde sakro-iliak ayrılmaların değerlendirilmesi: 56 olgu (2000-2003)

Bu çalışmada, klinik ve radyografik muayenelerle sakro-iliak ayrılma tanısı konan olgularda eş zamanlı komplikasyonların belirlenerek gerek konservatif ve gerekse farklı operatif tekniklerin geç dönem klinik sonuçlarının araştırılması amaçlanmıştır. Değişik ırk, yaş ve cinsiyetteki, 32 kedi ve 24 köpekte anamnez bilgileri, klinik ve radyografik muayeneler ışığında belirlenen 48 unilateral ve 8 bilateral sakroiliak ayrılma ile bunlarla eş zamanlı şekillenen yumuşak/sert doku yaralanmaları çalışma kapsamına alındı. Olguların 32'si operatif (20 kedi ve 12 köpek), 6'sı hem operatif hem konservatif (4 kedi ve 2 köpek) ve 15'i konservatif (7 kedi ve 8 köpek) yöntemle sağaltıldı. Diğer olgulardan, bir kediye genel durumunun desteklenmesi için devam eden sağaltım sürecinde öldüğü, iki köpeğe ise operasyona getirilmediği için müdahale edilememiştir. Bazı olgularda müdahaleyi takiben ve geç dönemde hematüri, konstipasyon, parezi, vida gevşemesi ve buna bağlı gelişen pelvis kanalı daralması gibi komplikasyonlarla karşılaşıldı. Çalışma sonrasında farklı nedenlerden ötürü değerlendirilemeyen olgular dışındakilerden 44 olgunun yürüyüş fonksiyonlarının normal olduğu 4 olgunun ise çeşitli derecelerde topalladığı saptandı. Sakro-iliak ayrılmanın çeşitli yumuşak/sert doku lezyonlarıyla birlikte ortaya çıktığı, sağaltım aşamasında ve öncesinde bunun göz önünde bulundurulması gerektiği ayrıca uygun büyüklükteki tek vida uygulamasının gerekli stabilizasyonu sağlayacağı sonucuna varılmıştır.

Evaluation of sacro-iliac separatöons in dogs and cats: 56 cases (2000-2003)

In the present study, by defining the accompanying complications at the same time, it was aimed to investigate the late stage of the clinical results of either conservative or different operative technique in the cases diagnosed with sacro-iliac separation by clinical and radiological examinations. 32 cats and 24 dogs with various breed, age and sex were included in the study. History, clinical and radiological examinations revealed that the animal material had 48 unilateral and 8 bilateral sacro-iliac separation accompanying with soft/firm tissue injury. 32 of the cases were treated by surgical intervention (20 cats and 12 dogs), 6 of the cases treated by both conservative and surgical technique (4 cats and 2 dogs) and the 15 of the. cases treated by conservative methods (7 cats and 8 dogs). From the remaining animal, one cat died due to poor healthy condition and no intervention was made for the other two dogs since they were not bring to operation by their owner. Various complications such as haematoma, constipation, paresis, screw releasing and narrowing of the pelvic canal depending on screwing were encountered in some cases following the operation or late stage of the operation. Over all the cases, 44 animals had normal gait pattern and 4 cases had various degree of lameness. The remaining cases could not be assessed clinically due to various reasons. In conclusion, sacro-iliac separation occurred by various degree of soft/firm tissue injury which should be taken into consideration before and during the treatment. Moreover, a proper size of screw application would provide the stabilisation.
