Effective Mood Stabilization with Olanzapine Monotherapy in an Adolescent with Treatment Resistant Rapid Cycling Bipolar Disorder
Erken başlangıçlı iki uçlu bozukluğun tedavisinde mizaçdüzenleyicileri ve atipik antipsikotiklerin birçoğu kliniktekullanılsa da, gençlerle yapılan randomize kontrollü çalışmaların sayısı kısıtlıdır. Ayrıca akut iki uçlu depresyon vesürdürüm tedavisini içeren ileriye dönük çalışmalar da çokaz sayıdadır. Bu yazıda hızlı döngülü iki uçlu bozukluğuolan ve hem akut manik dönemde hem de sürdürümtedavisinde olanzapin tedavisine iyi cevap veren 16yaşında bir ergen sunulmaktadır. Olanzapin ergenlerdekiiki uçlu bozukluğun hem akut döneminde hem sürdürümtedavisinde etkili bir seçenek olarak görünmektedir. Ayrıcamanik belirtilerdeki iyileşme, tedavinin ilk günlerindebaşlamaktadır.

Most of mood stabilizers and antipsychotic agents arecommonly used for early-onset bipolar disorder inclinical settings although there is limited number ofrandomized controlled trials with youths. Also, there arefew prospective studies for the treatment of depressiveepisodes in bipolar disorder and maintenance therapyin children and adolescents. Here we report a 16 years- old adolescent with rapid cycling bipolar disorder whoresponded well to olanzapine monotherapy both at theacute manic phase and during the maintenance treatment.Olanzapine seems to be an effective choice duringboth acute phase and maintenance treatment of bipolardisorder in adolescents. Also, improvement in manicsymptoms appears in the early days of treatment.
