Sitomegalovirus Enfeksiyonuna Bağlı Ciddi Trombositopeni
Sitomegalovirüs (CMV) enfeksiyonu immün sistemi normal olanlar bireylerde genellikle asemptomatik seyretmekle birlikte nadiren ciddi klinik tablolar tanımlanmıştır. Sağlıklı erişkinlerde CMV enfeksiyonu sonrası ciddi trombositopeni gelişen vakalar bildirilmiş olmasına rağmen çocuklarda bildirilen vaka sayısı azdır. CMV enfeksiyonuna bağlı trombositopenide tedavide ilk seçenek steroidlerdir, bununla birlikte dirençli vakalarda intravenöz immunglobulin (İVİG) tedavisi oldukça etkili olmaktadır. Biz bu yazıda CMV enfeksiyonu sonrasında trombositopeni gelişen ve İVİG tedavisine dramatik yanıt alınan bir çocuk hastayı sunduk.AbstractCytomegalovirus (CMV) infection is generally asymptomatic in the immunocompetent hosts and rarely defined as a severe clinical situation . However there have been several case reports of trombocytopenia after CMV infections in adults, there are few reports about pediatric cases. The first choice in trombocytopenia related with CMV infections is streroids, in refractory cases intravenous immunoglobulin (IVIG) therapy is very effective. In this article we describe a pediatric patient who has trombocytopenia after CMV infection and be dramatically treated by IVIG therapy.
