SAĞLIK HİZMETİ SUNUMUNDA HASTA – HEKİM İLETİŞİMİNİN HASTA MEMNUNİYETİNE ETKİSİ: BİR KANO MODEL ANALİZİ
Hasta hekim iletişimi kişiler arası bir süreçtir ve tıbbi bakımın ana kompenenti olarak ele alınmaktadır. Hasta hekim ilişkisinde hasta hekime varoluşsal bir bağlılık içindedir. Hastanın hekime güven duyması, tedavinin etkinliği için birincil koşuldur. Araştırmamızda sağlık hizmeti sunumunda hasta-hekim ilişkisini tanımlayıp, iletişim modelleri, tarihsel süreç, iletişimin pozitif ve negatif dinamikleri ve hastaların iletişim sürecine bakış açısı ele alınmıştır. Bir eğitim ve araştırma hastanesinin acil servisinde yaşanan hasta-hekim ilişkisi örnek olay olarak alınıp analiz edilmiştir. Elde edilen verilerden yola çıkılarak kalite geliştirme aracı olan ‘Kano modeli' kullanılarak hasta gereksinimleri saptanıp önceliklendirilmiştir. Çalışmamızın sonucunda hastaların hekiminden memnuniyeti ya da memnuniyetsizliğine yol açan en önemli etkenin hekimin güleryüzlü davranması ve hastaya yapılması planlanılan tanı ve tedavi hakkında yeterli ve anlaşılır bir şekilde bilgi vermesi olduğu görülmüştür. Bu durumda hasta memnuniyetini sağlamanın ilk adımı hastanın en doğal hakkı olan bilgi alma hakkına riayet edilmesidir.
THE EFFECT OF PATIENT-PHYSICIAN COMMUNICATION TO THE PATIENT SATISFACTION IN HEALTH SERVICE: A KANO MODEL ANALYSIS
Patient-physician communication is a course between individuals and is the main component of medical care. Patients have a interdependence to the doctor in this communication. The primary state for the effect of the therapy is trust. In the study, we aimed to define the patient-physician communication and assess the models of communication, the historical course, positive and negative dynamics of communication and patients' view to the communication. With the information which had been collected, we defined and revealed the priorities of the patients using the Kano model, a development tool of quality. In conclusion, it was revealed that good-humored behaviour of the doctor and giving enough and understandable information to the patient about the diagnosis and therapy were the most important factors causing the satisfaction or dissatisfaction of the patients. So, the primary step to achieve the satisfaction of the patient is to respect the patients' right of receiving information, a naturel right.
