DOĞUMSAL LOBER AMFİZEM: BİR OLGUNUN SUNUMU
Doğumsal lober amfizem, akciğerin bir veya birkaç lobunun aşırı havalanması ile karakterize, genellikle bebeklik döneminde solunum sıkıntısı ile bulgu veren nadir durumdur. Direkt akciğer grafisi genellikle çok yardımcıdır ve etkilenen lobda havalanma artışı, buna bağlı komşu lobların baskılanması, karşı tarafa mediastinal şift, aynı taraf diyafragmanın düzleşmesi ve etkilenen tarafta kot aralarının genişlemesi şeklinde görülebilir. Doğumsal lober amfizem, cerrahi bir hastalık olarak kabul edilir ve tüm semptomatik olguların lobektomi ile tedavi edilmesi önerilmektedir. Ayırıcı tanıda dev büllöz amfizem, diğer doğumsal akciğer malformasyonları, pnomotoraks ve yabancı cisim aspirasyonu gibi birçok doğumsal ve edinsel hastalık düşünülmelidir. Bu sunuda, solunum sıkıntısı ile başvuran ve lobektomi ile başarılı bir şekilde tedavi edilen 11 aylık bir bebekte doğumsal lober amfizem olgusunu sunmayı amaçladık.
CONGENITAL LOBAR EMPHYSEMA: A CASE REPORT
Congenital lobar emphysema is rare disease, characterized by hyperinflation of the one or several lobes of the lungs, and usually presents in infancy with respiratory distress. Chest X-ray is usually very helpful, and many of signs can be seen such as hyperinflation, suppression of adjacent lobes, mediastinal shift to the opposite side and the flattening of the diaphragm can be seen on the affected side. Congenital lobar emphysema is considered to be a surgical disease and recommended a lobectomy in all symptomatic patients. Differential diagnosis of giant bullous emphysema, other congenital malformations of the lung, pneumothorax, and many congenital and acquired diseases, such as foreign body aspiration should be considered. Herein, we aimed to present a case of congenital lobar emphysema in a 11 months old infant who presented respiratory distress and successfully treated by lobectomy.
