YAĞ GREFT İLE MİRİNGOPLASTİ

Miringoplasti, timpanik membran perforasyonlarını onarmak için kullanılan cerrahi prosedürdür. Son 40 yıldır timpanoplasti operasyonlarında temporal kas fasyası en sık kullanılan greft materyalidir. Bununla beraber tragal perikondrium, periosteum, yumuşak cilt altı dokusu, yağ ve dura da kullanılmaktadır. Kliniğimizde Haziran 2000- Haziran 2003 tarihleri arasında 15 hastada yağ greftli miringoplasti uygulandı. Bu hastaların 5’i posttravmatik persistan perforasyon, 10’u kronik otitis media sekeli olup, bunların 3’ünde timpanoskleroz mevcuttu. Hastalar kulak zarı perforasyonları 2-4 mm arasında ve santral olanlardan seçilmiş olup, lokal anestezi ile kulak lobulünden alınan yağ greftler, perforasyon kenarları avive edildikten sonra delik içine kum saati şeklinde yerleştirildi. Postoperatif en az 3 ay en fazla 36 ay otoskopi ve pür ton odiyometri ile takip edildi. Postoperatif otoskopik olarak hiçbir hastada perforasyon görülmedi. Odiyolojik olarak anlamlı bir işitme değişikliği saptanmadı. Timpanosklerozlu olguların orta kulak patolojileri ikinci seansa bırakıldı. Kuru kulaklarda küçük perforasyonlarda, yüksek başarı oranı, postoperatif çok düşük morbiditesi ve ayaktan uygulanabilmesi ve nedeniyle emin bir yöntem olduğu görülmüştür

FAT PLUG MYRINGOPLASTY

Myringoplasty is a surgical procedure to repair tympanic membrane perforations. The fascia of the temporalis muscle is the most common greft used for tympanoplasty during the last 40 years. Besides this; tragal perichondrium, periosteum, stromal soft tissue, fat tissue and dura had been used. Myringoplasty with fat tissue has been realised at our clinic during June 2000 and June 2003 at 15 cases. Five were posttravmatic persistant perforations, 10 were sequela of chronic otitis media, and there were tympanosclerosis at 3 cases. The cases, who have central perforations with a width of 2-4 mm were elected to our study. The fat tissue obtained from the ear lobule were intoduced in place after the aviviation of the perforation sites. The cases were followed with otoscopy and pure tone audiometry at the 3 rd and 36 th postoperative months. There were no perforations at the cases postoperatively. The were no stastically significant difference in pre and postoperative audiological findings. The middle ear pathologies of the cases with tympanosclerosis were designed for the second stage. Myringoplasty with fat tissue is an easy method for closing the small perforations of the dry ears. It is also an outpatient procedure with low morbidity
