Akut böbrek yetmezliği

Akut böbrek yetmezliği ABY glomerüler filtrasyon hızında GFR gelişen azalma ve bunun sonucunda kan üre azotu, kreatinin ve diğer üremik toksinlerin vücutta birikimini ifade eden bir durumdur. GFR’deki düşüşün aylar hatta yıllar içerisinde geliştiği kronik böbrek yetmezliğinin aksine, akut böbrek yetmezliğindeki GFR düşüşü daha hızlıdır ve günler ile haftalar içeriside gelişir 1 . GFR’deki azalma önceden herhangi bir renal hasarı olmayan bireylerde oluşabileceği gibi, önceden kronik bir renal bozukluğu olan bireylerde de akut ekzazarbasyon şeklinde ortaya çıkabilmektedir. ABY’nde idrar miktarı değişkendir. Sıklıkla oligüri günde 400 ml’den daha az idrar ve anüri günde 100 ml’den daha az idrar ile birliktedir, ancak nadiren de olsa idrar miktarında azalma olmayabilmektedir nonoligürik ABY . Total anüri nadirdir ve bu durumda akut kortikal nekrozdan şüphelenilmelidir.
