Arnold Chiari-I Malformasyonu ile Muhtemel İlişkili İnternükleer Oftalmopleji

Arnold Chiari-I malformasyonu (ACM) serebellar tonsillerin spinal kanala doğru herniasyonu ile karakterize bir konjenital anomalidir. Farklı klinik ve zamanlarda çeşitli semptomlar görülebilir. 40 yaşında bir kadın hasta akut başlangıçlı yürüme güçlüğü, bulanık ve çift görme ile kliniğimize başvurdu. Nörolojik muayenesinde internükleer oftalmopleji saptandı. Bu yazıda; ACM bulgusu olarak bulanık görme, baş ağrısı ataksi ve sağ INO ile başvuran, dekompresyon cerrahisinden sonra, sadece kısmen sola bakışta sağ gözde içe bakış kısıtlılığı devam eden nadir bir vakayı sunduk.

Internuclear Ophthalmoplegia Probably Related To Arnold-Chiari Malformation

The Arnold-Chiari-I malformation (ACM) is a congenital anomaly characterized by herniation of the cerebellar  tonsils into the spinal canal. The anomaly may present in a variety of ways and at times with vague symptoms. A 40-year-old female patient referred to our clinic with complaints of acute onset walking diffuculty, blurred vision, headache and diplopia. Neurological examination revealed right internuclear ophthalmoplegia. We present the unusual case of a patient suffered from blurred vision, ataxia and right INO as a finding of ACM which improved with only, at the right eye introspection remained restricted to left, after decompression surgery.
