İnsan fötusunda kalp kapakları gelişiminin ve aralarındaki ilişkinin ultrasonografi ve diseksiyon yöntemleri ile araştırılması

Amaç: insan fötuslarında kalp kapaklarının gelişiminin ultrasonografi ve diseksiyonla belirlenerek gestasyonel yaşla (GY) korelasyonunu araştırmak. Yöntem: Komplikasyonsuz ve tek fötuslu 48 gebede fötus kalp kapakları gelişimi prospektif olarak ultrasonografi ile değerlendirildi. GY'ları 23-39 hafta arasında değişen fötuslarda mitral, triküspit, aorta ve pulmoner kapak çapları ölçüldü. Ayrıca Anatomi Anabilim Dalı fetus kolleksiyonundan alınan ve GY'ları 14-40 hafta arasında değişen spontan abort 70 adet fötusta kalp kapakları ölçüldü. Ölçüm verileri ile GY arasındaki korelasyon değerlendirildi. Ayrıca, diseksiyon verileri cinsiyete göre karşılaştırıldı. Bulgular: Kalp kapakları gelişimi ile GY arasında pozitif korelasyon saptandı. Ultrasonografik verilere göre aorta ve triküspit kapak gelişim hızı, diseksiyon verilerine göre ise pulmoner kapak çapı haricindeki tüm parametrelerde gelişim hızı 2. trimestrda daha fazla bulundu. Pulmoner kapak çapı ile GY arasındaki korelasyonda trimestrlar arasında anlamlı bir farklılık saptanmadı. Ölçümler, cinsiyete göre bir farklılık göstermedi. Sonuç: insan fötus kalp kapaklarının gelişimine ait veriler, fötal kalp gelişiminin değerlendirilmesi ve konjenital kardiyak defektlerin doğum öncesi tanısında yararlı olabilir.

An investigation on the development of the valves of human fetal heart and their relationship by ultrasonography and dissection

Objective: Determination of the developmental pattern of the valves of human fetal heart by ultrasonography (USG) and dissection, and investigation of the correlation with the gestational age (GA). Methods: Development of the valves of the heart was evaluated in 48 pregnant women with uncomplicated single pregnancy prospectively by USG. GA was ranged from 23 to 39 weeks, and the diameter of mitral, tricuspit, aortic and pulmonary valves were measured. in addition, the diameter of the cardiac valves was measured in 70 spontaneously aborted human fetuses obtained from the fetus collection of Anatomy Department. G A was ranged from 14 to 40 weeks. The correlation was evaluated between measured values and the GA. in addition, the dissection data was compared regarding to sex. Results: There was a positive correlation between the development of the valves of heart and GA. The rate of increase in the diameter of the aorta and tricuspit valves measured by USG, and all parameters apart from the diameter of pulmonary valve measured by dissection were significantly higher in the 2nd trimester. There was no difference regarding to correlation ratio of trimesters between the pulmonary valve diameter and GA. The measurements did not show any difference regarding to sex. Conclusion: The values of the development of the valves of fetal heart may help the evaluation of fetal cardiac development and in the prenatal detection of congenital cardiac defects.
