Çok dedektörlü bilgisayarlı tomografi ile kardiyak değerlendirme

Amaç: Bu derleme son yıllarda hızla gelişen ve her geçen gün klinik kullanımı yaygınlaşan ÇDBT koroner anjiyografinin teknik özelliklerini, uygulama yöntemini ve tanıdaki başarısını göstermek üzere hazırlandı. Ana bulgular: Koroner arter hastalığı gelişmiş ülkelerde en önemli ölüm sebebidir. İnvaziv koroner anjiyografi koroner arterlerin anatomi ve patolojisini değerlendirmede, koroner arter hastalığında koroner lümen daralmalarının büyüklük ve şiddetini göstermede halen standart teknik olarak kullanılmaktadır. Çok dedektörlü BT (ÇDBT) primer olarak obstrüktiv koroner hastalıkları değerlendirmede kullanılmasına rağmen, konjenital defektler ve koroner arter anomalilerinin saptanmasında, torasik girişimlerin planlanmasında, bypass greftlerin ve koroner stentlerin değerlendirilmesinde ve kardiyak fonksiyonların gösterilmesinde giderek artan bir şekilde kullanılmaktadır. Çeşitli BT teknikleri ile yapılan koroner arter kalsiyum skorlama koroner arter hastalığı riskini saptamada kullanılmaktadır. Sonuç: Bilgisayarlı tomografi (BT) tekniğindeki yeni gelişmeler aterosklerotik koroner hastalığın ve diğer kardiyak patolojilerin noninvaziv bir şekilde değerlendirilmesine olanak sağlamıştır.

Cardiac evaluation with multi-dedector computed tomography

Objective: This review will focus on the basics of the scanner, the application methods and the diagnostic performance of MDCT coronary angiography as a rapidly improving diagnostic modality with an expanding clinical use. Main findings: Coronary artery disease is the leading cause of death in the advanced countries. Invasive coronary angiography remains the standard for assessment of coronary anatomy and pathology, and for determining the extent and severity of coronary lumen obstruction in coronary artery disease. Although evaluation of obstructive coronary disease is the primary use of multi-dedector CT (MDCT), its use in identifying congenital defects, coronary artery anomalies, planning thoracic procedures, evaluating bypass graft and coronary stent patency, and characterizing cardiac function continues to grow. Coronary artery calcium measurements have been used with different CT techniques for determining the risk of coronary events. Conclusion: Recent advances in computerized tomographic (CT) techniques of the heart allow for accurate, noninvasive characterization of atherosclerotic coronary disease and other cardiac abnormalities.
