Zayıf Yapılı Bireylerde Meme Onarımı için Transvers Miyokutanöz Grasilis Flebinin Doku Yeterliliğinin Değerlendirilmesi
Amaç: Meme kanserinin görülme sıklığı her geçen gün artmaktadır ve tanı almayaşı düştükçe, otojen rekonstrüksiyonun altın standardı kabul edilen karındokusu temelli fleplerin uygun olmadığı hasta sayısı da artmaktadır. Farklı birdonör saha arayışına girildiğinde, uyluk iç yüzünden alınacak transversmiyokutanöz grasilis (TMG) kas-deri flebinin yeterli doku sağlayabileceğidüşünülmüştür. Karın dokusu donör saha olarak elverişli olmayan, zayıf yapılı,doğum yapmamış bireylerde, uyluk iç yüzündeki doku yeterliliğinin objektifolarak ortaya konması hedeflenmiştir.Gereç ve Yöntem: Çalışmaya zayıf yapılı, kadın gönüllü hastalar dahil edilerektoplam 116 hasta incelendi. Katılımcıların yaşı, boyu, kilosu, doğum hikayesi vesütyen ölçüleri sorgulandı ve vücut kitle indeksleri (VKİ) hesaplandı. Dokuyeterliliğinin objektif bir parametre ile değerlendirilmesinde cilt kaliperikullanılarak cilt altı yağ dokusu kalınlığını ortaya koyan pinch kalınlıkları karın veiç uyluk bölgelerinde ölçüldü. Katılımcılar sütyen cup ölçüsüne göre üç grubaayrıldı ve meme boyutu, VKİ ve iç uyluk pinch değerleri arasındaki ilişkiistatistiksel olarak incelendi.Bulgular: Katılımcıların yaş ortalaması 24.7 yıl, ortalama VKİ 20.63 idi. The Evaluation of Tissue Sufficiency Regarding the Transverse Myocutaneous Gracilis Flap for Breast Reconstruction in Thin Individuals
Objective: For autologous breast reconstruction, the flaps of choice are mainly centered around abdominal tissue as the primary donor site. Yet with the increase in newly diagnosed young, nulliparous breast cancer patients, the abdomen may sometimes not be a suitable option as donor tissue. The transverse myocutaneous gracilis (TMG) flap has become a popular alternative for breast reconstruction candidates in this patient group. The purpose of this study was to evaluate the tissue sufficiency of the medial thigh regarding breast reconstruction in thin individuals. Material and Methods: The study was conducted on 116 female volunteers with a slim body habitus. The age, height, weight, history of pregnancy and breast cup sizes were questioned for each patient and body mass indexes (BMI) were calculated. A skinfold caliper was used to objectively evaluate the tissue thickness of the proximal medial thigh and the lower abdomen. Participants were categorized into three groups according to their breast cup size and the relationship between breast size, BMI, and thigh pinch thickness were evaluated.
