Bir Sığırda Özefagus Kaynaklı Metalik Yabancı Cisim Sebebiyle Oluşan Derin Boyun Apsesi ve Sağaltımı
Bu olgu sunumunda boyun bölgesinde şişkinlik, iştahsızlık, regürgitasyon ve zayıflama şikayetleri bulunan 3 yaşındaki Simental ırkı bir inekte gözlenen özefagus kaynaklı metalik yabancı cisim nedeniyle oluşan derin boyun apsesi ve sağaltımı konu edildi. Radyografik muayenede boyun bölgesinin derin dokularında metalik bir yabancı cisim bulunduğu ve cismin yakınında lokalize olmuş radyolusent görüntü veren apse bulgularına rastlandı. Endoskopik muayene sonucunda normal özefagus bulgularına rastlanırken özefagus lümenin dorsomedialinde apse basıncı ile oluşan parsiyel daralma gözlendi. Boynun sol tarafındaki şişkinlik üzerinde deri ensizyonu ile punksiyon gerçekleştirildi ve boyun kasları arasından dren uygulamasıyla apse içeriği boşaltıldı ve daha sonra metalik yabancı cisim çıkarıldı. Gazlı bezle uygulanan drenler birer hafta arayla 3 kez değiştirildi. Ayrıca drenler her gün 4-5 cm kesilerek kısaltıldı. Hasta 8 hafta sonunda tamamen iyileşti. Başarılı sağaltım için semptomatik sağaltım yerine tanı yöntemlerinden yararlanılmasının önemi bu olgu sunumuyla bir kez daha vurgulanmaktadır.
Deep Cervical Abscess Caused by Eosephageal Metallic Foreign Body and its Treatment in a Cattle
In this case report, deep cervical abscess due to oesephagal metallic foreign body and its treatment were reported in a 3-years-old Simmental cow exhibiting swelling in cervical area, anorexia, cachexia and regurgitation. Radiographic examination indicated that there was a metallic foreign body together with translucent abscess formation. In the endoscopic examination, it was observed that normal appearance of oesophagus with partial luminal structure caused by abscess pressure. Skin incision and punction were performed on the swelling of left neck region, and content of the abscess was discharged with drain application among the muscles, and then the metallic foreign body was removed. Gauze drains were changed 3 times at weekly intervals. Also, the drains were shortened by cutting 4-5 cm each day. At the end of 8 weeks, the lesion completely healed. This case report emphasizes once again the importance of using diagnostic methods instead of symptomatic treatment for successful treatment.
