Periodonsiyumun sekonder hastalandığı primer endodontik lezyonlu dişlerde tedavi: vaka raporu

Primer endodontik lezyonlu dişler sekonderolarak apikal foramen, furkasyon, lateral ve aksesuar kanallar vasıtasıyla periodontal lezyonlarasebep olabilir.Karmaşık periodontal rejeneratif cerrahiişlemlere gerek duyulmaksızın sadece cerrahiolmayan periodontal tedavi ve endodontik tedavi ilebu tip dişlerin tedavi edilebileceğini bir vaka raporuile sunmak amacımızı oluşturmuştur.38 yaşında bayan hasta kliniğimize santraldişinde renklenme sorunu ile başvurdu. Radyografik muayene sonucu ilgili dişte apikal ve laterallezyon tespit edildi. 4 mm cep derinliği kaydedildi.Santral ve lateral kesici dişlere endodontik tedaviuygulandı. Tek seans küretaj ve kök düzlemesiyapıldı. Tedavi sonrasında cep derinliği 2mmolarak ölçüldü. Radyografik inceleme sonucunda12.ayda lezyonun büyük ölçüde kaybolduğugörüldü.Vaka tedavisi sonucu primer olarak endodontik lezyonlu periodontal sorunlu dişler endodontiktedavi ve cerrahi olmayan periodontal tedavi ilebaşarılı bir şekilde tedavi edilebilir inancındayız.Tedavi yöntemini seçerken sorunun kaynağını saptamak ise oldukça önemlidir

Treatment of Teeth with Secondarly Diseased Periodontal Lesion Due to Primary Endodontic Lesions: A Case Report

Teeth with primarily endodontic lesions cancause secondary periodontal disease because of theapical foramen , furcations, lateral and accesorycanals.The aim of this case report is to present resolution of combined lesion with conventionalendodontic and conservative periodontal therapywithout the need of complicated regenerative periodontal surgery. A 38 years old woman referred to our clinicwith discoloration of central incisor teeth.Radiographic examination revealed an extendedapical and lateral lesion. Pocket depths of 4 mmwere measured. Both the central and lateral incisorteeth were endodonticaly treated. Periodontal treatment-scalling and root planning was performed onall affected teeth within one appointment. Pocketdepths of 2mm were measured after the treatment.Radiographic examination showed that the lesionwas dissolved in 12 months to a considerableamount.As a conclusion, periodontally diseased teethwith primary endodontic lesions can be successfully treated with conventional root canal treatmentand conservative periodontal treatment. The originof the disease is of superior importance in choosing the treatment modality
